  • KDA_4_20211A2 - --One lid in use in reach in cooler in back kitchen, covering cocktail size hot dogs with crack in lid. One lid stored as clean on shelving above 3 vat sink with severe cracks in rim. 2 plastic containers (one holding cinnamon/sugar mix the other holding yeast) with cracks in rim and in interior of container. Not COS
  • KDA_4_50111B - --Seal to right reach in cooler in back kitchen is not intact. Hinge on left cabinet door to cabinet under faucet/sink in front area is not intact.
  • KDA_4_60111A - --On shelving above 3 vat sink stored as clean in back area, one metal pan with old date marking sticker residue on surface as well as 3 plastic containers that previously held syrups for lemonades with old sticker residue on the surface. Lid to lemonade mixer machine soiled with mold buildup on surface.
  • KDA_5_20515B - --Handsink in back kitchen leaks from handles when in use. When hot water is turned on, the hot water knob leaks. When the cold water is turned on, both leak.
  • KDA_6_20214 - --Door to unisex toilet room in back kitchen area, for use by employees, is not self closing and hangs open.
  • KDA_6_30114 - --No handwashing signage at handsink in back room at 3 vat sink or in toilet room.
  • KDA_7_20111A - --Sanitizer bucket stored in front area on shelf next to and in splash zone of single use cups to be used to hold RTE foods prepped in establishment. Corrected on-Site COS moved
