KDA_3_30414B1 - --The wet wiping cloth bucket is showing 0ppm chlorine upon testing. The owner said she had not yet added bleach to it. COS - she added bleach to the bucket to 100ppm.
KDA_6_20215A3 - --There is a narrow 1/8 to 1/4 inch gap along the side of the front door where sunlight can be seen shining through while the door is closed.
KDA_6_20215D - --The owner has the back door open to allow the cool morning air inside. There is no screen door or other form or protection to prevent the entry of pests at this door.
KDA_6_30114 - --There is no hand washing sign posted at the hand sink in the prep area. Corrected on Site (COS) - provided a sign to be posted.
KDA_6_50111 - --There are broken/chipped floor linoleum throughout the retail area. The floor is chipped/broken in the back storage room.
KDA_7_10211 - --There is a red bucket in the sink with a clear liquid and rag inside. The owner identified this as her bleach water for sanitizing. COS - she labeled the bucket.
KDA_7_30111B - --On the retail shelf there is a bottle of isopropyl alcohol, peroxide, calamine lotion, and Elmer's glue sitting on a shelf directly above bottled beverages. COS - the owner moved these items upon request.
KDA_Defacto - --Dasani Cooler: Ham - 41.6 F Coke Cooler: Milk - 37.4 F --There are opened two opened containers of lunch meat in the Dasani cooler that the owner said she opened this morning and yesterday. There is a container of cut and cleaned lettuce that the owner said she prepared yesterday. --There are six trash cans outside. Two of them have lids. The owner said the food/boxes/anything relating to food only goes into the trash cans with lids.
KDA_KSA65_657a - --On the retail shelf, the following drugs were expired: Ibuprophen - Expired 10/13, Lot 2628 (1 box of individual packets) Cold Relief - Expired 1/15, Lot 2638 (1 box of individual packets) Advil - Expired 3/15, Lot R10172 (2 individual boxes) COS - all were removed from the retail shelf by the owner.
