All items were date marked past 7 days. COS-marked all of the items with the corrects dates.

  • KDA_2_30114F - --The cook wore gloves while cracking raw hard shell eggs, then continued to handle the squirt bottle of butter with the same gloves. The cook removed the contaminated gloves and then placed new gloves on without handwashing. COS-instructed to wash and educated.
  • KDA_3_10111 - --At the bar there was one bottle of Jim Bean Bourbon with 5 small flies in the product. There was one bottle of Jack Daniels with 2 small flies in the product. There was one bottle of Captain Morgan Original Spiced Rum with one small fly in the product. COS-discarded. In the Cafe RIC, there was an open gallon of chocolate milk and an open gallon of 2% milk without known dates. COS-discarded. In the Kitchen Cafe RIC (below MT 2) there was an open package with 2 turkey sausage patties without a known date. COS-discarded.
  • KDA_3_30211A1b - --In the Kitchen Cafe make table 2 there was a metal food grade container of raw turkey bacon being stored directly with ready to eat sausage links and ready to eat sausage patties. In the RIC (below the MT) there was raw beef being stored directly above ready to eat chicken stock and egg whites (pasteurized). There was also ready to eat smoked brisket being stored directly next to raw tuna. There was ready to eat crab meat being stored directly next to raw steak. COS-rearranged the cooler.
  • KDA_3_30212 - --In the Kitchen Cafe, above the grill there was a squirt bottle with clear liquid in it and not labeled. The person in charge said it was water.
  • KDA_3_30412B - --In the Chinese area, there was a large bin of rice with a plastic measuring cup without a handle and in direct contact with the product. There was also a metal bowl without a handle being stored in direct contact with flour. COS-removed both. In the Kitchen WIC there was a black bowl with out a handle in direct contact with baked beans. COS-removed.
  • KDA_3_30511A1 - --In the Kitchen/Deli WIF-there was one box of bread being stored directly on the floor.
  • KDA_3_50116A2 - --In the Deli make table there was a squirt bottle of ranch dressing at 47.1F and a squirt bottle of Caesar dressing at 44.4F. They have been in the unit longer than 4 hours, the bottles contents were above the container fill line. The ambient air temperature of the unit was reading 34.6F. COS-discarded.
  • KDA_3_50118A1 - --Kitchen Cafe-in the RIC (below MT 1) there was an open jar of garlic and oil (keep refrigerated) with an open date of 12/20/15 (held past 7 days). There was also 4 portion sized bags of wild rice date marked 12/29/15-1/14/16 (held past 7 days). COS-discarded. In the Kitchen WIC there was a large plastic food grade tub of pulled pork date marked 12/27/15 (held past 7 days). There was also a large food grade tub of baked beans dated marked 12/31/15 (held past 7 days). COS-discarded.
  • KDA_3_50118A3 - --Kitchen Cafe-in the RIC (below MT 1) there was a plastic food grade container of Buffalo sauce (made on site) date marked 1/3/16-1/10/16 (past 7 days). COS-put the correct date on the container.
  • KDA_4_20211A2 - --On the utensil rack in the Kitchen Cafe ware wash area, there was one plastic lid with cracks. COS-discarded. In the utensil drawer in the Deli area, there was one rubber spatula with cracked edges. COS-placed aside to trim the edges smooth. In the Chinese area, there was one plastic scoop for the rice that had a melted edge. COS-discarded.
  • KDA_4_50213A - --In the Chinese area, in the container of sesame seeds there was a single use plastic bowl being used to scoop the product out. COS-discarded
  • KDA_4_60111A - --In the Kitchen Cafe ware wash area, on the utensil rack there were 2 (18 qt) and one (22 qt) plastic food grade containers with stickers/residue left on them and stored clean. There was one plastic serving tray with a sticker on the bottom and stored clean. There was one rubbermaid plastic food grade container with sticker residue left on it and stored clean. In a cabinet to the right of the meat slicers, there were 2 plastic lids and one plastic container with stickers left on them and stored clean. In the Italian area, there were 2 knives and a pizza cutter with dried food debris and stored clean. In the back storage area there were 2 knives with food debris on the blades and stored clean. On the utensil rack there were 3 (1/3) metal food grade pans with dried food debris and stored clean. In a plastic food grade container of peppers there was a plastic scoop with a sticker on the handle. In the Kitchen Cafe area, there was one metal spatula with silver duct tape around the handle. COS-discarded. COS-all items were moved to the ware wash area.

  • KDA_4_90311B - --In the back storage area on the utensil rack there were 3 round metal bowls that were stacked wet/clean. There was also 3 rectangle metal pans stacked wet/clean (not able to properly air dry). COS-moved to the ware wash area.
  • KDA_4_90411B - --On the counter there was a plastic utensil holder with metal forks facing upwards. There was also a utensil holder in the ware wash area that had metal forks facing upwards. COS-inverted.
  • KDA_5_20515B - --The spray nozzle faucet at the 3 compartment sink located at the Kitchen Cafe ware wash area leaks when water is supplied.
  • KDA_6_30112 - --There were no disposable paper towels available at the handsink behind the bar. COS-placed some at the handsink.
