KDA_3_30212 - --Located in the make table there was a pan of yellow crumbs that did not have a label. PIC said it was cooked chopped eggs.
KDA_3_30412B - --Located in the preparation area there is one container of coffee grounds with a cup being used for the scoop and it does not have handles.
KDA_4_50111A - --The brown RIF has a broken lid and broken gaskets and the lid is crooked and not tight fitting. There is an ice accumulation on the inside of the unit.
KDA_4_60211E4 - --The ice machine guard plate has visible black mold on the edges and sides.
KDA_5_50116C - --The kitchen hand sink does not have a trash can close to the sink for placing used single use towels.
KDA_5_50117 - --The FE has 2 unisex toilet rooms. Both rooms do not have a cover on the trash can.
KDA_6_20214 - --The FE has 2 unisex toilet rooms. Both rooms do not have self closing doors.
KDA_6_20215A3 - --The back door and the front door both have gaps in the bottom and sides and are more than 1/4 inches.
KDA_6_50111 - --The ceiling tiles in the kitchen area and preparation area are all broken, warped and are sagging. PIC said they did have one incident where the tiles fell out of the ceiling and repairs are supposed to be done soon.
KDA_6_50112A - --The floor area in the maintenance room has an accumulation of dirt and trash debris. The floors along the baseboards have an accumulation of dirt debris. The wall vent behind the ice machine has an accumulation of lent debris.
KDA_7_10211 - --Located at the mop sink, there were 2 spray bottles of clear liquid with no labels. PIC did not know what they were. COS. Items were dumped down the sink. Located on the make table, there was a container of clear liquid with a wiping cloth inside and no label. PIC said it was sanitizer. COS. Container was labeled.
KDA_7_20111B - --Located on top of the make table, there was one container of sanitizer sitting directly above RTE cooked eggs and RTE cooked chicken. COS. Container was moved. Located on the rack for juice and water there was one spray bottle of sanitizer hanging on the shelf directly above the Orange juice. COS. Bottle was moved. Located in the storage room there were two bottles of sanitizer and one bottle of lotion sitting directly above 2 bags of coffee grounds. COS. Chemicals were moved.
KDA_Defacto - --3 vat sink uses chlorine for the sanitizer. Chlorine concentration was 50ppm. --Critical (Critical)
