KDA_2_30114H - --Observed a employee that did not wash his hands before putting on gloves. He was wrapping food containers in plastic wrap with bare hands. Then he went to put on gloves without washing hands. COS PIC was educated. Employee washed his hands.

KDA_3_50118A2 - --In the produce WIC, ranch dressing that is made on site that contained buttermilk did not have a prepared or discard date. PIC said it was 10/13 around noon. COS Container was labeled.
KDA_4_60111A - --Stored as clean, white plastic containers were stacked with sticker residue on the exterior surface. COS Containers were removed to be washed, rinsed, and sanitized.

KDA_Defacto - --PIC said he stop all ROP. He has submitted a HACCP plan 3 weeks ago and waiting for a decision.
