KDA_4_90311A - --Three containers full of clean scoops and utensils were stored in plastic containers in deli with accumulation of dried food debris and soil. (corrected on site-all scoops, utensils and containers were moved to dishwashing area) KDA_6_50112A - --Floors and drain behind soda machine have build-up of soil and trash. KDA_Defacto - --3-vat sink had a reading of 200 ppm quaternary and facility has quaternary test strips. --A sausage pizza with a temperature of 133 and a pepperoni pizza with a temperature of 128 was stored in hot box in deli. A steak burrito stored in another deli hot box had a temperature of 121 and held for 1 hour according to paper used to log time and temperatures. Establishment uses time as temperature control and pizzas were marked with time and temperatures. Facility disposes of foods after 2 hours. --Chili and cheese reheating in chili/cheese dispenser for 1 hour had a temperature of 120.
