- KDA_3_50116A2 - --Located in the sandwich RIC in the retail area, there were two Italian subs at 53F, two turkey bacon swiss subs at 53F, two ham and cheese subs at 51F, four Sub Jr. at 54F, four chili dogs at 59F, three ham and cheese sandwiches at 51F, four pork ribs sandwiches at 49F, three chuck wagon sandwiches at 50F, and four big daddy sandwiches at 51F. Person in charge indicated that these sandwiches were stocked yesterday. Corrected on site (COS)- discarded, educated on proper cold holding temperatures.
- KDA_4_20211A2 - --Located in a cabinet under the kitchen counter, there was one plastic food container lid and six plastic food containers that were pitted. COS- discarded.
- KDA_4_30111 - --The sandwich RIC located in the retail area had an ambient temperature of 44F. COS- discarded all potentially hazardous foods.
- KDA_4_50111B - --The seal on the Montgomery Ward chest freezer is broken.
- KDA_4_60111C - --The tracks of the Nestle chest freezer at the back of the kitchen next to the retail area have an accumulation of dust and debris on them. The fan guards of the WIC are dusty. The inside of the cream and sugar station machine in the customer self serve area has an accumulation of sugar and sweetener powder.
- KDA_4_60211E4 - --There is mold on the ice chutes and under the soda nozzles of both of the customer self serve soda fountains. There is mold on the inside rim of the ice machine located in the storage room.
- KDA_4_90311A - --Located in the storage room, there were two boxes of single use cups and a box of single use chicken to go boxes stored on the floor.
- KDA_4_90411B - --Located at the customer self serve microwave at the end of the front counter, there were cups of forks, knives, and spoons that were stored with the food contact surface facing upwards.
- KDA_5_501113 - --The door to the outside dumpster was open.
- KDA_6_50111 - --There are several missing floor tiles on the floor next to the fryer. There is chipping paint on the wall behind the kitchen hand sink. There is chipping paint on the wall in the storage room. There area several ceiling tiles in the storage room that are stained. There is chipping paint on the shelves of the storage room.
- KDA_6_50112A - --There is debris on the floor behind the front counter and on the floor behind the fryer. The floor along the walls of the storage room has an accumulation of dust and debris. There is an accumulation of dust and debris on the floor under the three vat sink. There is mold on the ceiling of the WIC. There is debris on the floor of the WIC.
- KDA_7_10211 - --Located at the end of the kitchen counter, there was an unlabeled container of clear liquid which the PIC identified as sanitizer. COS- labeled.
- KDA_7_20111B - --There was a container of sanitizer stored on a shelf above the clean dish rack at the three vat sink. COS- moved sanitizer.
- KDA_Defacto - --fried potato wedges 210F --hot case- chicken tenders 180F, chicken wings 146F, --kitchen reach in cooler (RIC)- bacon 40F, burrito 38F, WIC- chocolate milk 40F, milk 38F, --Located in the WIC, there was a small container of gravy at 83F at 12:30pm. At 2:00pm, it was at 52F. Person in charge (PIC) indicated that it had been put in the WIC at 11:30 this morning after breakfast. --test strips available --wiping cloth QT sanitizer concentration- 200ppm (per label)
