KDA_3_50118A1 - --In the walk in cooler, an opened container of commercially prepared salsa, was date marked 10/27-11/4. Salsa was held more then seven days. COS-salsa was discarded. KDA_3_50118A2 - --In the walk in cooler, an open gallon of milk was with out a date. Employee said it was opened yesterday. COS-milk was dated with correct date. KDA_4_20211A2 - --In the stack drawers on the east wall, there were two rubber spatula's that had "scrapper" wrote on them. Three rubber spatula's had pits and chips on the edges of them. One metal spoons handle was melted. COS-these items were moved to another drawer where they will not use them. KDA_4_204112B - --The milk cooler is with out a thermometer. KDA_4_60111A - --Located in a drawer under the make table, two slotted ladles were stored as cleaned with dried on food debris. COS-ladles were placed at mechanical warewashing machine. The commercial can opener had dried on food debris on the cutting blade. KDA_6_20215A3 - --The back north exit door has a 1/4" gap along the bottom left side. KDA_6_50111 - --The walk in freezer, has an accumulation of ice build up on the interior left side of the door and the exterior right side of the door. There is a hole that goes through the wall from the mechanical ware washing area into under the dry storage shelving in the kitchen. The paint is off the wall behind the sink at the mechanical ware washing area. The paint is coming off the cabinet front on right upper cabinet door on the east wall. KDA_6_50112A - --There is black mold on the exterior door and the bottom metal edge of the walk in freezer. There is an accumulation of dust on the, ceiling, fan covers, light covers, vent covers, cabinet tops and the pipes on the ceiling. There is dirt and debris on the ledge of the stove front. KDA_7_10211 - --The sanitizing cloth bucket was with out a common name. COS-bucket was labeled correctly. KDA_Defacto - --Frozen food was frozen. --Walk in cooler, cooked carrots 38.1F.
