  • 5-202.14 - --On the mop sink in the janitor's closet just off the dining area, the faucet has a built in backflow prevention device (atmospheric vacuum breaker). The device has failed and leaks when the water is turned on. A hose used for filling mop buckets was hooked to the faucet and was also hanging below the flood rim of the sink (removed). This hose must remain off of the faucet at all times until this device is repaired or replaced.
  • KDA_4_60111A - --On a shelf stored as clean, five single use plastic food container lids and 12 single use plastic food containers, all stored as clean and intended for re-use, have stickers on the outside surfaces (removed to office for disposal or cleaning later).

  • KDA_5_20515B - --A water pipe is leaking onto the floor just east of the dishwasher. No hose was hooked to this faucet and no hose can be hooked to this faucet until this device has been repaired or replaced. This was cited during the last inspection on 21 August 2015 and has not been fixed.

  • KDA_4_204112B - --No visible thermometer in the south milk cooler (the built in thermometer is inoperable).
  • KDA_4_60111A - --One ladle stored as clean had masking tape on the handle (cleaned). Also, 11 single use plastic food container lids and one single use plastic food container, stored as clean, and intended for reuse had remains of stickers and adhesive on the outside surfaces (discarded).
