- 5-202.14 - --In the maintenance closet there is a mop sink with an Atmospheric Vacuum Breaker (AVB) installed. There is a black hose attached to the faucet downstream of the AVB. The black hose is hanging below the flood rim of the mop sink. When the water is turned on, water leaks from under the bell of the AVB. The hose was removed today to provide an air gap. The kitchen manager said she would have an order put in to have this AVB fixed.
- KDA_3_30611 - --There is a cooler of ice on a cart in the dining area. It is available for volunteers to serve customers, and customers walk up to this cart to get a drink. The cooler does not have a lid or other form of protection and is exposed to every customer walking by. COS - the kitchen manager covered the cooler with plastic wrap until she can replace it with a cooler that has a lid.
- KDA_4_60111A - --There are four lids and three containers that have sticker residue on the outside of the containers in the kitchen. These are all being stored as clean. Corrected on Site (COS) - the kitchen manager took these to ware washing to be cleaned.
- KDA_7_10211 - --There are two buckets in the kitchen that are labeled as Multi-Purpose Calibrated Storage Containers. They are not labeled to identify what precisely is stored inside of them. The kitchen manager verified they have sanitizer in these buckets. COS - she labeled these.
- KDA_7_20111A - --There is a spray bottle of sanitizer stored directly next to boxes of aluminum foil on the counter in the kitchen. COS - the kitchen manager moved the sanitizer. There are Clorox wipes and hand soap stored directly next to tea, straws, salt, pepper, coffee, and plastic drinking cups in the cabinet in the dining area. COS - the kitchen manager moved these. There is dance floor wax stored on a shelf directly next to single-use napkins and cups in the commodities closet. COS - the kitchen manager moved this.
- KDA_7_20111B - --There is a spray bottle of sanitizer on top of a prep table and above roasters on the bottom shelf of the prep table in the kitchen. COS - the kitchen manager moved this. There is a bucket of sanitizer sitting on top of a prep table in the kitchen and above trays on the bottom shelf of this prep table in the kitchen. COS - the kitchen manager moved this. There is dance floor wax directly above single-use napkins and cups in the commodities closet. COS - the kitchen manager moved this. In the kitchen restroom there is a spray can of Lysol Disinfecting spray stored directly on top of the paper towel dispenser. COS - the kitchen manager moved this.
- KDA_Defacto - --Cooking Chicken Pot Pie - 188.1 F --Steam Table: Chicken Pot Pie - 141.8 F --The kitchen manager provided thermometers but expressed concern about them not reading right despite being calibrated regularly and often seeming off even right after calibrating. Provided the kitchen manager with a digital min/max thermometer and demonstrated its use. --There is Raid Flying Insect Spray (EPA Reg 4822-513) on the shelf in the maintenance closet. The kitchen manager said this is not used and was left by a previous employee, she just has not had time to clean out the unused chemicals in the closet. --Turbo Cooler (Kitchen): Cheese - 43.2 F Dining Floor Cooler: Milk - 39.9 F Refrigeration Room: True Cooler: Cheese - 44.8 F (Note - kitchen manager said they have been in and out of this cooler quite a bit this morning) White Cooler: Cheese - 40.4 F

- 5-202.14 - A backflow or backsiphonage prevention DEVICE installed on a WATER SUPPLY system shall meet American Society of Sanitary Engineering (A.S.S.E.) STANDARDS for construction, installation, maintenance, inspection, and testing for that specific application and type of device.
- KDA_3_30611 - FOOD protected - Except for nuts IN the Shell and Whole, Raw fruits and vegetables that are intended for hulling, peeling, or washing by the CONSUMER Before consumption, FOOD on display shall be protected from contamination by the use of packaging- counter, service line, or salad bar FOOD guards- display cases- or other effective means.
- KDA_4_60111A - Food Contact Clean to Sight and Touch - EQUIPMENT FOOD-CONTACT SURFACES and UTENSILS shall be clean to sight and touch.
