As per the kitchen manager, it was opened last Saturday (5 December 2015)(dated). In the make table, two containers of open commercially prepared ready to eat sausage patties not date marked. As per the kitchen manager, the sausage was opened last Saturday (5 December 2015)(dated).
- KDA_4_20211A2 - --Repeat violation - one plastic food container and one plastic food container lid are cracked (both discarded). This prevents proper cleaning and pieces of plastic can end up in foods.
- KDA_4_60111C - --Repeat violation - The reach in freezer in the kitchen has mold and soil on the bottom door gasket and gasket area.
- KDA_6_501111A - --New violation - in the kitchen, one live roach was crawling up the wall near the grill. As per the kitchen manager, they have a hired pest control service that sprays for insects and sets out sticky traps. Pest control method observed was sticky traps.

- KDA_3_50114A1 - --At 3:39 pm, in a reach in cooler, chili that was cooling was 99 degrees. The manager stated it went in to the cooler at about 2 pm. At 4:14 pm, the chili was 88 degrees. It did not cool down to 70 degrees or less within two hours (discarded). Also, the chili was in in a crock pot container with a lid, and was also in a home style cooler. Reviewed proper cooling procedures with the manager and suggested using one of two commercial coolers for cooling foods.
- KDA_3_50118A1 - --In a reach in cooler, open commercially prepared ready to eat brats not date marked. As per the manager, they were opened November 16th. They were not discarded within seven days (discarded).
- KDA_3_50118A2 - --In a make table, ready to eat lettuce cut yesterday and put in the cooler more than 24 hours ago and open commercially prepared ready to eat sausage opened yesterday and more than 24 hours ago not date marked. Dated both items (more than 24 hours ago as per manager).
- KDA_4_10119 - --In the make table, bare wood slats are being used as spacers and to hold food containers up.
