Note: manager said they hired a licensed pest control operator on 8/13/15 and they came out to spray the same day. They also baited areas and put traps out. KDA_6_50112A - --There are dead insect on the floor behind office desk, below the ware wash area and in the restroom. KDA_7_20111B - --There was a working spray bottle of Spic & Span being stored directly on top of the Sanyo RIC. COS-removed chemical. KDA_7_20212A2 - --There was a jug of Ortho Home Defense Insect Killer: EPA Reg. No. 279-9534-239 being stored in the establishment and it is not approved for restaurants.COS-discarded. KDA_8_40411 - --There were approximately 16 live cock roaches in the food establishment in the back storage room. They do have a licensed pest control provider and they are on a monthly routine, last inspection is unknown.
