5-202.14 - --The atmospheric vacuum breaker leaks when water is supplied at the mop sink. (Note: the hose was cut above the flood rim). KDA_3_10111 - --In the bun hold below the hot dog roller machine, there was one hot dog bun with green mold growing on the outside. COS-discarded. KDA_4_60111C - --There is dust build up on the fan blowers in the True reach in cooler that hold sandwiches. KDA_4_60211E4 - --Both Cappuccino machines have dried food debris build up inside of the plastic dispensers. The left Cappuccino machine has dried food debris on the inside of the door. KDA_6_20211A - --The light fixture above the slushy machine does not have a protective cover. KDA_6_20215A3 - --There is a 1" gap at the left bottom corner at the back exit door. There is also a 1/4" gap along the left side of the door. This could allow pests to enter. KDA_6_30114 - --There was no handwashing sign in the public restroom. COS-left one handwashing sticker. KDA_6_50111 - --There are broken floor tiles inside of the walk in cooler. The left side of the back exit door frame is broken and deteriorating. The seal around the back exit door is torn along the left side and top. KDA_6_50112A - --There is dried food debris inside of the cabinet below the slushy machine. There is trash on the floor inside of the walk in cooler. KDA_7_30111B - --On a retail shelf there was one Tide To-Go instant stain remover being stored directly on top of a Hershey's chocolate bar. COS-removed chemical. On a retail shelf there were 12 bottles of liquid dish soap, 4 packages of Cascade dishwasher detergent being stored directly above 7 boxes of Kleenex and 2 packages of single use plastic cups. There was also 2 cans of Glade spray and Lysol disinfectant spray and 3 cans of Comet with bleach being stored directly above 5 packages of single use foam cups. COS-removed chemicals and rearranged shelf.
