  • KDA_2_30114I - --An employee who was cutting meat in the meat department had on gloves and lifted his ball cap off of his head and then replaced the ball cap. He continued to work with the meat without changing gloves or washing his hands. COS, educated employee when handwashing need to occur.
  • KDA_2_30411 - --The meat processing employee has blood on the bill of his ball cap.
  • KDA_3_20215 - --There were 4 cans of food on the retail shelves that had dents in them. COS, VD.
  • KDA_3_30211A1b - --In the open air meat case a package of raw brats were stored in direct contact with RTE imitation crab meat. COS, brats moved.
  • KDA_3_30511A1 - --In the WIF there are many boxes of food stored directly on the floor. There are bags of onions and boxes of tomatoes stored directly on the floor below the potato display. There are boxes of candy beneath the Halloween display.
  • KDA_3_50116A1 - --In the deli hot case the bbq chicken had a temperature of 117 F and the mozzarella sticks 114 F. After the PIC turned the temperature up the bbq chicken had a temperature of 145 F, the mozzarella sticks 128 F. COS, mozzarella sticks discarded. PIC said they were put in the case at 9:30.
  • KDA_3_50116A2 - --In the open air cheese case packages of cheese ranged from 44 F to 65 F. At 2:50 the PIC said the last time the case was stocked was at 10:30 AM. COS, cheese discarded. In the bacon bunker packages of hot dogs and bacon ranged from 48 F to 54 F. At 3:45 PIC said it was stocked at 10:30. The bunker was overstocked. COS, VD of packages on top. In the salad bar a container of lettuce temped 51 F, sliced tomatoes 48 F, cottage cheese, potato salad, macaroni salad, and tuna salad temped 44 F. PIC said items had been there since yesterday 9/14. COS, VD. In the open air packaged salad case packages of lettuce ranged from 44 F to 52 F. At 4:30 the PIC said it was stocked at 3:00 PM. The case is overstocked. COS, the packages of salad were moved to the WIC.
  • KDA_3_50118A1 - --In the deli case a package of ham was date marked 9/06. COS, VD. In the WIC an opened 1/2 gallon container of milk was date marked 9/03. COS, VD.
  • KDA_3_50118A2 - --In the WIC by the deli a container of nacho cheese was not date marked. PIC said it was opened on 9/10. COS, dated.
  • KDA_3_60211C - --There is no ingredients list posted for customers to view for the donuts in the self service case.
  • KDA_4_20211A2 - --Food storage containers in the deli are cracked. COS, VD. Meat trays in the meat department are cracked.
  • KDA_4_204112B - --There is not a thermometer in the hot boxes for the deli or in the salad bar.
  • KDA_4_30111 - --The salad bar is not holding PHF's at 41 F or below. Ambient 44 F The open air cheese cases are not holding the cheese at 41 F or below. Ambients 47.8 F, 44.1 F.
