- KDA_3_30211A1b - --Located in the RIC, there was one pan of raw chicken sitting on the shelf above 1 pan of RTE soup. There was one pan of raw tuna sitting on the shelf above RTE slaw. COS. Raw items were moved to bottom shelving.
- KDA_3_30412B - --Located inside the tempura powder and rice containers, the scoops were laying down with the handles touching the foods. COS. Scoops were washed, dried, and placed back inside properly.
- KDA_3_30412F - --Located at the cooking area, the scoops were being stored in a pan of water at 77.4F. COS. Scoops were washed, dried, and placed back inside properly.
- KDA_3_30414B1 - --Located in the cooking area there was one wet wiping cloth laying on the cutting board and not inside a sanitize solution.
- KDA_3_60211B1 - --Located in the cooking area there were 3 pans of white powder substance sitting beside each other with no label. PIC said one was salt, one was MSG, one was sugar.

- KDA_4_60211E4 - --The ice machine guard plate has visible purple mold on the edges.
- KDA_6_50112A - --The floors in the cooking area under the cooking equipment and sinks have an accumulation of black and sticky residue.
