COS - the assistant manager had this moved. In the customer service area of the store there is a spray bottle of food grade lubricant stored directly next to/against spray bottles of floor cleaner and glass cleaner. COS - the assistant manager had this moved.

- 4-302.14 - --The establishment uses quaternary ammonia as a sanitizer on their dishes. The quaternary ammonia test strips at the 3 vat sink in the deli are expired as of 11/1/15.
- 3-302.11(A)(2) - --On the retail floor in the meat cooler there is raw chicken on a shelf pushed over so that the end of the chicken package is hanging directly above raw pork brats. COS - a meat department employee rearranged this. --This is not cited as a violation: raw salmon was stored above raw pork in the meat department walk-in-cooler. Recommend using the silo method to separate different types of raw animal foods such as beef, fish (including shellfish) lamb, pork, and poultry. Please be aware that raw fish, including shell fish, pose a high risk for allergen cross contact and must be stored so it can’t contact other foods.
- KDA_3_30211A1b - --In the meat department walk-in-cooler there is a box of packages of raw salmon that are being stored directly on top of jarred ready-to-eat Herring and jarred ready-to-eat cocktail sauce. Corrected on Site (COS) - the assistant manager rearranged the cart. On the retail floor in the meat cooler there is raw sausage directly above fully cooked smoked sausage. COS - a meat department employee rearranged this.

- KDA_4_60111A - --The cheese slicer in the deli has food residue remaining on the back of the blade. The deli employees said that this slicer has not yet been used today and should be clean.
- KDA_4_90311A - --In the bakery there is a box of bread bags and a box of domed containers stored directly on the floor.
- KDA_4_90311C - --On a shelf in the deli ware washing area there are single-use plastic containers and lids that are outside of their original protective packaging, neither covered not inverted.
- KDA_5_20212A - --The hand sink in the produce prep area is only reaching 76.6 F. The employees in this area indicated that they commonly have problems with the hot water at this hand sink. They are able to temporarily wash their hands in the bakery/deli hand sinks in the neighboring department area and walk through the swinging doors back to the produce room without touching the doors.

- 3-302.11(A)(2) - Cross contamination by Raw Animal FOOD type - FOOD shall be protected from cross contamination by separating types of Raw animal FOODS from each other such as beef, FISH, lamb, pork, and POULTRY during storage, preparation, holding, and display by: using separate equipment for each type- arranging each type of FOOD in equipment so that cross contamination of one type with another is prevented- or, preparing each type of FOOD at different times or in separate areas
