The PIC stated the chicken had been on the counter for 1.5 hours. COS-moved chicken back to the cooler. KDA_4_10211A1 - --4-102.11(B)(1) Materials that are used to make SINGLE-SERVICE and SINGLE-USE ARTICLES Shall be Safe.---- Several non food grade (grocery store) bags are being used to store zucchini, raw chicken, and raw beef in the WIC and WIF. The food was in direct contact with the plastic. Advised PIC to move food to food grade plastic. KDA_4_20211A2 - --A spatula stored as clean had cracks and chips in it. COS-PIC discarded KDA_4_60111A - --A scoop used to portion out fried bread crumbles had a large amount of old encrusted food on it. COS-PIC cleaned scoop KDA_4_60111C - --Most of the equipment in the kitchen needs cleaned. There is a large accumulation of dried and old food/dirt on the prep tables and wok station. The frigidaire RIC had roach feces on the back panel. Advised to clean. KDA_4_60211E4 - --The soda nozzles have a large accumulation of biofilm slime in them. Advised to clean. KDA_5_20515B - --The North handwashing sink drains slow and backs up into the basin of the sink. The South handwashing sink drain pipe leaks. KDA_6_20213B1 - --Two fly strips with numerous dead flies were above the food prep table. COS-removed fly strips. KDA_6_30112 - --The North handwashing sink did not have paper towels. COS-supplied KDA_6_501112 - --There was a dead roach sitting on the South handwashing sink. COS-PIC removed. KDA_6_50112A - --The floors in the kitchen have a large accumulation of old food along the wall and floor junction. There is a foul smell by the three compartment sink. KDA_7_10211 - --Under the woks, a spray bottle with a purple liquid lacked the common name. The PIC identified it as a degreaser.
