- KDA_3_50116A2 - --In the middle MT, diced tomatoes 46F, salsa 44F, shredded lettuce 54F, cut lettuce 57F, and sliced tomatoes 67F had measured temperatures above 41F. PIC said he took the items out the WIC 2 hours ago. The temperatures were measured at 3:00. The ambient temperature was 57.3F. COS The diced tomatoes, sliced tomatoes, salsa, and lettuce was put on ice for rapid cool down. Recommended that the PHF should remain on ice until the MT can be look at by a repairman.
- KDA_4_30111 - --The middle MT had an ambient temperature of 57.3F.
- KDA_4_60111C - --The toaster had a dried food residue on the exterior surface.
- KDA_4_60212B - --In the kitchen, the microwave still had dried food on the interior and exterior surface including the ceiling.
- KDA_6_501112 - --Observed dead roaches on the side of the soda machine in the wait staff area. Contracted pest control company service visit is every Wednesday.

- KDA_3_10111 - --In the bar area, a bottle of Jim Beam Bourbon had small flies inside. COS The employee was educated. Contents of the bottle was discarded.
- KDA_3_30111B - --Observed employee fry RTE tater tots for a customer order. From the fryer, he dump the tots in a bowl to add seasoning. He then used his bare hand to put the tater tots on the customers plate. COS Employee was educated. Employee washed hands and put on gloves. Tater tots were discarded. While inspector was informing PIC about the MT issue, PIC used his bare hand to check the temperature of the RTE cut onions. COS PIC was educated. PIC washed hands and discarded onions.
- KDA_3_30212 - --Above the flat top stove on the right side were bottles of water and margarine that was not labeled with the common name.
- KDA_3_50114D - --In the glass RIC in the kitchen, raw scrabbled eggs had the ambient temperature of 54F. The glass RIC had an ambient temperature of 51.1F. PIC said the eggs were put in the RIC at 11:00am. The temperature of the eggs were taken at 2:00pm. COS The eggs were put in the WIC for rapid cooling.
- KDA_3_50116A2 - --In the glass RIC, cooked chicken wings 52F, raw chicken 54F, cut lettuce 52F, blue cheese 52F, and feta cheese 53F had temperatures measured temperatures at 2:00pm. PIC said it was put in the RIC at 11:00am. COS The PHF was taken out the RIC and place in the WIC for rapid cooling. In the middle MT, diced tomatoes 50F, buffalo sauce (contained butter) 50F, salsa 52F, shredded lettuce 55F, cut lettuce 59F, and sliced tomatoes 55F had measured temperatures at 1:30pm. COS The PHF was discarded. Cooked potatoes 88F and cooked green beans 84F were sitting in a double pan on top of the warming table. Employee said the food was put out at 11:30am. I ask the PIC was it suppose to be held hot or held cold? He informed inspector it should be held cold. COS PIC was educated. Potatoes and green beans were put in the WIC for rapid cooling.
- KDA_3_50118A2 - --At the pizza MT, a bag of commercially packed sausage and a container of diced ham did not have a prepare or discard date. PIC said the bag of sausage was opened 9/7. The chopped ham was cut 9/9 in the morning. In the middle RIC, cornbeef, roast beef, and sliced turkey did not have a prepare or discard date. PIC said the meat was prepared 9/9. COS The containers was marked with the date.
- KDA_4_30111 - --The glass RIC in the kitchen had an ambient temperature of 50.1F. PIC said the RIC was brand new, 1 day old. The middle MT had an ambient temperature of 48.1F PIC said he will call someone to repair the MT. Inspector suggested using ice to keep the PHF below 41F.
- KDA_4_60111C - --The toaster had a dried food residue on the exterior surface. In the WIC, there is food debris and standing water on the floor. There is a build up of grease on the walls in the kitchen and on the floor. Around the pull out drawer warmer that is underneath the hot holding warming station, was a build up of food crumbs and dried food residue.
- KDA_4_60212B - --In the kitchen, the microwave had dried food on the interior and exterior surface including the ceiling.
- KDA_4_90311A - --In the kitchen, 2 knives were stored in-between the middle MT and the pizza MT. COS Employee was educated. Knives were put in the warewashing area to be washed, rinsed, and sanitized.
- KDA_5_20511B - --In the bar area, in the basin of the handwashing sink was ice and straws. Employee said they have been using the handwashing sink as a dump sink. There were also tubes running from the bar area running into the sink.
- KDA_6_30111 - --In the bar area, there was no soap at the handwashing sink. Employee said they ran out of soap.
- KDA_6_30112 - --In the bar area, there was no paper towels at the handwashing sink.
- KDA_6_501112 - --Observed dead roaches on the side of the soda machine in the wait staff area. Employee said they were having a roach problem last month but have had a contracted pest control company visit every Wednesday. Pest control last visit was 9/9.
- KDA_6_50112A - --In the kitchen and the back prep area/warewashing area had food crumbs, greasy floor, and standing water.
