  • KDA_3_50115A - --REPEAT: There are two large pans of refried beans in the kitchen sitting out at room temperature. They are measuring 125.8 F. The owner said these have been sitting out for approximately 20 minutes. COS - the beans were put into ice baths and stirred regularly. Tried to have them put into smaller containers, but the owner said they do not have appropriate containers available for this at this time. There is guacamole sitting in the cold holding station near the steam table. It is measuring 47.8 F. It was made this morning around 1030am per staff. It is sitting on top of the ice rather than being down in the ice to cool properly. COS - the guacamole was put into the cooler.

These tomatillos are cooked. They were placed in this cooler yesterday evening. Raw Chicken - 53.1 F - Chicken was placed in cooler yesterday after being sliced Tomato puree - 53.9 F - was removed from a can yesterday and placed into cooler Chile Rellenos - 50.1 F - Placed in this cooler at approximately 11am this morning per staff (temperature was taken before 230pm) Cooked Potatoes - 44.2 F - Placed in the cooler this morning around 11am per staff (temperature was taken before 230pm) Diced Tomatoes - 44.5 F - Placed in cooler this morning at approximately 11am per staff (temperature was taken before 230pm). The tomatoes were diced cooled yesterday The ambient air temperature in this cooler is only reaching 45.9 F after remaining closed for approximately 40 minutes. COS - Items in the cooler since yesterday were discarded. The items that were put into this cooler from another cooler this morning were moved back to the other cooler. There is a cold holding station on the counter in the kitchen near the steam tables. The following items were not at proper cold holding temperatures: Shredded Lettuce - 51.4 F Shredded Cheese - 53.6 F Sour Cream - 48.1 F These items have been out since approximately 11am this morning. The temperature was taken before 1pm. COS - these items were moved into a cooler in the bar area. Explained to owner that the items are probably not staying cold because they are sitting on top of ice, rather than being down in the ice.

It has not reached proper cold holding temperatures by 8am this morning. It is in a thick plastic bucket with a large quantity of sauce inside. Let the staff know that the material of the bucket does not allow the heat to escape, and the amount of sauce in the bucket makes it harder for it to cool down.

When the dishwasher completed its cycle, he pulled the rack of clean dishes out and began touching them with the same gloved hands he had been handling dirty dishes with. COS - explained to him that he must wash his hands between dirty and clean dishes. He put these dishes through the dishwasher again.
  • 3-302.11(A)(2) - --REPEAT: In the "True" cooler there are two containers of raw chicken sitting directly on top of a container of raw beef. COS - the cook rearranged this.
  • 4-102.11(B)(1) - --REPEAT: There are corn tortillas on a plate above the steam table. The plate of tortillas is wrapped up in a plastic "Thank You" grocery bag. The grocery bag is in direct contact with the tortillas.
  • KDA_2_30112B1 - --REPEAT: The man doing dishes in the kitchen washed his hands with cold water only. He was washing his hands after handling dirty dishes, prior to putting clean dishes up from the dishwasher. Corrected on Site (COS) - stopped him and explained to him that he needs to be washing his hands with hot water. He washed his hands again.
  • KDA_3_10111 - --REPEAT: There are four containers of Flan in the salsa cooler that are not dated. The cook said he is unsure when these were cooked because he is not the one who does it. Upon checking the Flans, they were molding. COS - these were discarded.
  • KDA_3_30211A1b - --REPEAT: In the cooler below the make table there is a tub with a filet of raw tilapia on a shelf directly above a tub of ready-to-eat crab meat. COS - the tilapia was removed to be cooked.
  • KDA_3_30511A1 - --REPEAT: There are boxes of bagged soda pop on the floor below the bar. There are bags of sugar, salt, rice, and pinto beans on the floor in the dry storage area.
  • KDA_3_50114A2 - --REPEAT: In the "Coca Cola" cooler in the kitchen there is a large tub of cheese dip that is measuring 63.4 F. It is dated as being made yesterday (11/18). Upon speaking with the cook, he said the cheese has been in this cooler since yesterday. COS - the cheese was discarded.
  • KDA_3_50115A - --REPEAT: In the "Coca Cola" cooler in the kitchen there is a large blue container of cheese dip that was prepared yesterday and was placed in the cooler to be cooled.   • KDA_3_50118A1 - --REPEAT of 3-501.18: In the "Coca Cola" cooler in the kitchen there is a pan of Chipotle peppers that is dated 11/12. The cook said these are cooked peppers that come in a can. The date is when they were removed from the can. Explained to him that they had been held for one day too many and should have been discarded after the close of business yesterday (11/18). COS - they were discarded.
  • KDA_3_50118A2 - --REPEAT: In the "True" cooler in the kitchen there is a tub of cooked ground beef that has no date on it. The cook said they cook ground beef every two days and that this would have been from two days ago (11/17). COS - dated. In the "True" cooler in the kitchen there is a tub of Pico de Gallo that does not have a date on it. The cook said this was prepared yesterday (11/18), but he is unsure what time it was prepared. COS - dated.
  • KDA_3_60311B - --REPEAT: There is a consumer advisory posted on the final page of the menu. There is a reminder that consumer raw or under cooked items may cause illness,, but there is no disclosure stating what items they serve cooked-to-order. They have verified they do steaks cooked-to-order.
  • KDA_4_10111A - --REPEAT: There are multi-purpose containers in use in the kitchen and bar area storing food. The food items being stored in these containers include peppers, onions, raw beef, raw chicken, salsas, and chile rellenos. The food is in direct contact with the plastic of the container. These are Sterilite containers with no markings indicating they are safe for direct food contact. The beans and rice in the dry storage room are being stored inside of black trashcans. The trashcans are not labeled as being safe for food. The beans and rice are in direct contact with the plastic of the trashcans. Note - the manager said the owner has ordered new containers and they are waiting for them to come in.
  • KDA_4_20211A2 - --REPEAT: There are five chipped plates on shelves of clean dishes throughout the kitchen. There are two chipped bowls on a shelf of clean dishes in the kitchen. There are plastic containers storing peppers and onions in the "True" cooler in the kitchen. There are holes gouged around the upper edges of these containers. The holes appeared to have been manually done after the containers were purchased. They are not smooth and easily cleanable, and in some cases have bits of plastic hanging from them.
  • KDA_4_50119 - --An employee was washing the immersion blender manually at the spray head and garbage disposal in the kitchen. When tested, the water at the spray head is only reaching 101.7 F.

These stickers were identified as remaining from the original purchasing of the containers and have not been cleaned off. There are three containers on a shelf of clean dishes that have stickers left from the original manufacturer's purchasing of the containers after being cleaned. There are three plates in a stack of clean plates on a shelf in the kitchen. The plates have food splatter on them. COS - the man doing dishes took these to the dishwasher upon request. There are plates below the steam table that have fallen food debris on them. COS - the cook moved these plates to ware washing.
  • KDA_4_70211 - --The man doing dishes was cleaning the immersion blender. He scrubbed it with hot soapy water. He then rinsed it off and put it on a shelf of clean dishes. Upon speaking with him, he said he was done washing it and was doing no more to it. COS - explained that the dishes must be sanitized. Demonstrated how to make bleach water and use the test strips to verify the concentration. He took a clean cloth and sanitized the immersion blender by wiping it down with bleach water.

COS - the front employee moved these.

The tomatoes are measuring 49.2 F. The cook said these were sliced this morning by 9am. There is salsa in the salsa cooler that is measuring 46.7 F. The staff said this salsa was made this morning by 9am.

  • KDA_2_30112A - --The cook in the kitchen was washing his hands at the hand sink. He turned on the cold water, ran it over his hands for approximately five seconds, turned the water off, and began drying his hands. Corrected on Site (COS) - explained that this was improper hand washing and that he needed to wash his hands in hot water, with soap, for at least 20 seconds. Required he re-wash his hands.
  • KDA_2_30114G - --The cook in the kitchen put raw chicken on the grill with his gloved hands. He then went to the steam table, grabbed a clean plate, and began putting ready-to-eat chips on the plate with the same gloves. COS - stopped the cook and explained the hand washing violation. The chips were discarded and the plate placed in the dirty dishes area. The cook washed his hands and changed gloves.
  • KDA_2_30114I - --A kitchen employee left the kitchen. He returned (having to open a swinging door which he pushed open with his hand) and put on a pair of gloves and began handling raw chicken. COS - explained to him that any time he enters the kitchen he needs to wash his hands prior to food handling. He said he had washed his hands at the hand sink in the hall prior to entering the kitchen. Explained that since he had touched the door on his way in, hand washing was still required upon entering the kitchen. COS - he washed his hands. The chicken was not required to be discarded.

  • KDA_3_30511A1 - --In the hall by the back door there is an open bag of sugar. The sugar is uncovered and exposed to possible contamination. In the bar area there is a box of bagged soda pop sitting directly on the floor.
  • KDA_3_50114A2 - --There are several items in the windowed "True" cooler in the kitchen that were cooked yesterday and put into this cooler to be brought down to temperature. The kitchen staff said that these items have been in this cooler since yesterday, and that they have not been pulled out today, nor have they had to open and use this cooler much today. All items have been in this cooler since they were prepared yesterday morning - per staff. The temperatures have not yet reached 41 F. The temperatures were as follows: Cooked Pork for Carnitas - 50.4 F Cooked Pork for Carnitas - 47.7 F Cooked Chicken - 52.1 F Refried Beans - 47.7 F Refried Beans - 49.5 F Cooked Ground Beef - 50.9 F COS - all items were discarded for improper cooling. In the windowed "Coca Cola" cooler in the kitchen there is a large tub of queso. The cook said this cheese dip was made yesterday morning and put in this container and cooler to be cooled. The cheese is measuring 56.4 F. It has not been out of the cooler for extended periods of time since it was put in yesterday to cool. All other items in this cooler are within proper cold holding temperatures. COS - the cheese was discarded for improper cooling.
  • KDA_3_50115A - --There is pork for carnitas, chicken, ground beef, and refried beans that have been placed in the windowed "True" cooler to cool down after being prepared. These are all in large pans, covered, and stacked together. They are stored in a cooler where they are taking up approximately a third of the space and are likely raising the temperature of the entire cooler. COS - all items were discarded for taking longer than 6 hours to cool. There are cooked potatoes sitting out in a pan in the kitchen. They are measuring 97.0 F. The cook said these were cooked this morning around 10:30am. The temperature was taken approximately 1:45pm. The cook said these had been sitting out on the counter since being cooked. COS - let the cook know that cooling at room temperature is not an adequate cooling method and required the potatoes be put in the cooler.

All items were identified as having been in this cooler since yesterday or before. The staff said they had not had this cooler open very much this morning. There are several large pans of items that were placed in this cooler to cool after being cooked yesterday. COS - all items were discarded. There are items sitting out on ice in the kitchen. They are not measuring within proper cold holding temperatures. They are as follows: Pico de Gallo - 56.2 F Sliced Tomatoes - 59.7 F Hot Salsa - 60.6 F Chile Salsa - 58.9 F Shredded Lettuce - 47.4 F Shredded Cheese - 47.0 F Sour Cream - 51.6 F Guacamole - 61.7 F Staff said that these items had been taken out of the coolers and put on ice at 10am. It was approximately 150pm when the temperature was taken. COS - all items were discarded as they cannot be cooled before they will have been out of temperature for 4 hours. There is a salad bar near the bar area that has tubs of salsa inside. One tub of salsa (the hot salsa) is measuring 49.5 F. The staff said this had been here on ice since yesterday. COS - the salsa was discarded. In the small cooler with salsa for the tables there are 11 individual salsa containers that are measuring 49.1 F and 8 individual flan containers that are measuring 47.8 F. The staff said these items have been in this little cooler since yesterday. COS - these items were discarded and this cooler was turned down to a lower temperature.
  • KDA_3_50116B - --There is a box of eggs sitting out on the bottom shelf of a table in the kitchen. The eggs are measuring 75.2 F internally. The cook said these eggs have been here two days when he was asked. Questioned him again to verify that the eggs have been sitting out on the table with no temperature control for two days and he verified that they had been here the entire time and were used in items prepared for guests. COS - the eggs were discarded.
  • KDA_3_50118A2 - --In the windowed "True" cooler in the kitchen there are two pans of beans, one pan of chicken, and two pans of pork for carnitas that the cook said were all cooked cooked yesterday (11/3) in the morning. None of these items are date marked with when they were made. COS - These items were discarded. In the windowed "True" cooler in the kitchen there is an open gallon of milk that has no opening date. The cook said this was opened two day sago (11/2). COS - the milk was discarded. In the windowed "True" cooler in the kitchen there is a tub of cooked potatoes that are not dated. The cook said were cooked Sunday (11/1). COS - the potatoes were discarded. In the windowed "True" cooler in the kitchen there is a container of pico de gallo with no date marking. The cook said was made Monday (11/2). COS - dated. In the windowed "Coca Cola" cooler in the kitchen there is a plate of ready-to-eat tamales that are not dated. The cook said these were prepared yesterday (11/3) in the morning. COS - dated. In the windowed "Coca Cola" cooler in the kitchen there is a container of ready-to-eat poblano rellenos that are not dated. The cook said thee were prepared Monday (11/2). COS - dated. In the windowed "Coca Cola" cooler in the kitchen there is an opened package of ready-to-eat hot dogs that is not dated. The cook said that these were opened three days ago (Sunday - 11/1). COS - dated. In the windowed "Coca Cola" cooler in the bar area there is an open gallon of milk that is not dated. The front staff member said this milk was opened three days ago (Sunday (11/1). COS - dated. In the small cooler for salsa for the tables, there are 8 homemade flan containers. There is no date marking on these. The cook said these were prepared Sunday (11/1). COS - the flans were discarded.

These are found throughout the kitchen and the bar area. In the dry storage room there are two black trash cans that are being used to store rice and beans. The trash cans have no markings or labeling indicating they are safe for direct food contact. They are only labeled as being used for trash. These food items are in direct contact with the plastic of these trash cans.
  • KDA_4_20211A2 - --There are four chipped ceramic plates and 1 chipped ceramic bowl being stored with the clean dishes throughout the kitchen. COS - these were removed.
  • KDA_4_204112A - --The thermometers that have been placed in the coolers are all placed in the back of the cooler where it will be coldest. Letting staff know that these should be hanging in the front of the coolers where the temperature will be the warmest.
  • KDA_4_30111 - --The ambient air temperature of the windowed "True" cooler in the kitchen is only reaching 50.7 F. It is possible that the ambient air temperature in this cooler is out of temperature due to the large number of items placed in this cooler to cool down after being cooked. The cooler was turned down and the staff will monitor it for proper temperature.

COS - The prep sink in the kitchen was cleaned and converted to a hand sink. Let them know that if this door was not in place, the hand sink in the hall could be the kitchen hand sink, but removing this door is not entirely recommended because this is right next to the public restroom. Recommending another sink be installed in the kitchen that will be used expressly for hand washing so that the indirectly plumbed sink in the kitchen may be used for prep.
  • KDA_6_30112 - --The sink in the kitchen that is now being used for hand washing does not have a supply of paper towels. COS - the cook supplied a roll of paper towels to keep on the shelf next to the sink.
  • KDA_6_30114 - --There are no hand washing signs posted at the hand sinks in the kitchen, hallway, or restrooms. COS - provided with signs to be posted.
  • KDA_6_501111A - --There are two flies in the kitchen that are flying around the exposed chicken and diced tomatoes. The flies are landing on the edge of the container holding the raw chicken and crawling down the inside of the container slightly. They never landed directly on the chicken or cutting board it was being prepped on.
  • KDA_6_50112A - --There is food splatter, debris, and grease on the walls, floors, below cooking equipment, and below coolers throughout the kitchen.
  • KDA_7_10211 - --There i an unlabeled blue bucket on the floor in the dining area. A staff member verified that this was bleach water. Requested this be labeled - it was not done during the inspection.
  • KDA_7_20111A - --In the kitchen there is a bottle of dawn dish soap stored on the rack by the sink. It is directly next to cans of pineapple and bags of tortillas. This was moved when it was requested and set inside of the edge of the sink so it will still be convenient for hand washing, but it was later found sitting back beside the food product. There is bleach and dawn dish soap on the bottom shelf of the rack by the sink. These chemicals are directly next to bags of tortillas and a bottle of liquid margarine. COS - the chemicals were moved under the sink. In the dry storage room there is a bottle of bleach being stored directly next to Clamato Tomato Juice. Requested this be rearranged - this was not done during the inspection. In the dry storage room there was a bottle of hand soap directly next to cans of Chipolte Peppers and a box of sweetener packets. Requested this be rearranged - this was not done during the inspection. There is a spray can of insect killer directly next to the Lemon Sour Mix on the flour in the bar area.
  • KDA_7_20111B - --In the kitchen there is dawn dish soap sitting on a rack directly above bags of tortillas. This was moved upon request to the inside edge of the sink where it would still be convenient for hand washing, but it was later found sitting back on the rack with the food product. In the dry storage room there is a spray can of bug buster and an air freshener bottle sitting directly on top of cans of Chipolte Peppers. Requested this be rearranged - this was not done during the inspection.
