KDA_3_50118A2 - --Located in the RIC, there was one pan of beef wontons with no date. PIC said they were made on December 1. COS. Pan was dated. KDA_4_20211A2 - --Located at the make table, there was one cracked and chipped bowl being used as a food scoop. COS. Item was placed in trash. KDA_4_50111A - --The front counter has cracks in the sides and top and is not cleanable KDA_4_60111C - --The racks/shelving that are inside the WIC have an accumulation of food debris and rust. The microwave has an accumulation of grease debris on the door. The WIC vent covers, vent box, door, door handle, gaskets, ceiling and floor all have an accumulation of black debris and lent. The white RIF has an accumulation of black debris on the gaskets. The hand sink faucet and handles have an accumulation of food debris. Located inside the cabinets on the shelving in the self serve soda area, there is an accumulation of dried aged rodent feces and 3 dead roaches. KDA_4_60211E4 - --The ice chute at the soda station has visible pink mold on the ice contact surface. KDA_5_20515B - --The 2 faucets at the 3 vat sink have a constant drip. KDA_6_50112A - --The ceiling vents in the hallway by the toilet rooms, the vents in the dining area, and the vents in the kitchen area all have an accumulation of lent and grease debris. The floor of the WIF has an accumulation of food debris. The back door in the kitchen has an accumulation of black debris. The floor and ceiling in the storage room has an accumulation of black debris. The top of the WIC/WIF unit has an accumulation of lent debris.
