- 3-501.17(B) - --A package of sliced ham from the freezer was setting on the counter (still frozen). The inspector asked when it was sliced. The cook said that they slice the ham, wrap it, and freeze it. They then pull out the potions they need daily. The package was not dated for when the original package of ham was opened and sliced. Inspector spoke with them about dating the packages when they are sliced and placed in the freezer and when they pull it out of the freezer to date the package again and only hold it for 6 days.
- KDA_3_10111 - --On the prep table by the grill, pre-cooked sausage patties and raw bacon were in the same container and touching. Corrected on site (COS): Cook voluntarily discarded items in the trash can.
- KDA_3_50116A2 - --There was a plastic container on the prep table by the grill that had raw bacon (67.8F) and pre-cooked sausage (59.3F), items were not maintained at 41F or below. COS: Cook voluntarily discarded them in the trash. In the Roper Refrigerator 2 dozen eggs (50.0F), sliced cheese (46.7F), and cream cheese (48.9F) were not maintained at 41F or below. COS: Items were voluntarily discarded in the trash.
- KDA_4_10111A - --By the grill, there was a non-food grade plastic container that had raw bacon and pre-cooked sausage in direct contact with the container. On the backroom wire storage shelving, the non-food grade containers used for panko and flour had a liner in them, but they were not food grade liners (trash bags).
- KDA_4_30111 - --The Roper Refrigerator (53.7F) is not maintaining 41F or below for PHF. DO NOT USE THIS REFRIGERATOR FOR PHF UNTIL IT CAN MAINTAIN 41F OR BELOW.

- KDA_6_20211A - --The light fixture above the prep table by the grill is not shielded.
- KDA_6_20215A3 - --The back door has a small gap at the bottom right that could allow for pest entry.
- KDA_6_50111 - --Throughout the establishment there are water stained ceiling tiles and some are sagging.
- KDA_6_501111A - --Behind the estate refrigerator in the waitress area, a glue board had 1 live roach on it. Behind the water heater, a glue board had 1 live roach on it.
- KDA_6_501111B4 - --On the wire storage shelving there were pieces of cardboard cut to fit on the shelves. Inspector educated about the potential for pest harborage.
- KDA_6_501112 - --There are several glue boards throughout the establishment that have dead roaches on them.
- KDA_6_50112A - --There is grease build up on the floor behind the grill.
