KDA_3_10111 - --Located in the RIC there were several PHF food items being held without a date and PIC did not know when foods were placed in cooler. sliced ham, sliced beef, sausage gravy, pork and beans. COS. Foods were VD into trash can. KDA_3_30211A1a - --Located in the RIC, there was one container of raw chicken sitting on top of one container of raw beef. COS. Chicken was moved. KDA_3_30212 - --Located in the RIC there was one pan of yellow liquid with no label. PIC said it was raw egg wash. COS. PIC decided to VD item. KDA_3_30412B - --Located inside the flour container the scoop was laying in the food with the handle touching the food. COS. PIC removed scoop and rewashed it. KDA_3_50118A2 - --Located in the RIC, there was one pan of raw egg wash with no date. PIC said it was made on November 21. COS. PIC decided to VD the wash. KDA_4_20211A2 - --Located on the shelving for clean dishes, there was one cracked plastic container. COS. Item was placed in trash can. KDA_4_60111A - --The mixer attachment used for mixing food items has an accumulation of dried food debris. KDA_4_60111C - --The RIC has a large accumulation of sticky debris, dried food debris on the bottom shelving, interior, exterior, door and door handle. The WIC has a large accumulation of sticky black debris, dried food debris on the floor, walls, ceiling, interior, exterior, door and door handle. The mixer base has a large accumulation of dried food debris. The tubing and floor around the soda syrup has an accumulation of sticky debris. The vent hood filters have an accumulation of lent debris. KDA_4_90311A - --Located in the storage room, there are 2 cases of single use cups sitting on the floor. Located on the shelf in the storage room, there was a soiled scoop sitting on the shelf directly above a tube of single use soufflé cups. PIC said the cups are used for food and the scoop is used outside for scooping ashes. COS. Cups were placed in trash and scoop was moved. KDA_5_20511B - --The hand sink in the kitchen cooking area is blocked with an empty beer keg, which has to be moved to reach the sink to wash hands. KDA_6_30112 - --The hand sink in the bar area did not have any hand drying supplies at the sink. COS. PIC found a clean cloth towel which was used only once. KDA_6_50112A - --The floor area in the storage room has an accumulation of dirt and aged dried rodent feces. The floor areas in the kitchen, bar, and storage room in kitchen have an accumulation of black sticky debris. KDA_7_20111A - --Located in the bar area there was a stack of dinner napkins sitting beside 1 bottle of sanitizer and 1 bottle of hand lotion. COS. Napkins were moved. KDA_7_20111B - --Located in the storage room, there were 5 bottles of hand soap sitting on the shelf directly above 1 case of single use cups.
