- 3-302.11(A)(2) - --Raw chicken was stored on a wire shelf in the WIC above raw pork. Raw chicken was stored on a wired shelf above raw shrimp in the WIC.
- KDA_3_30211A1b - --In the WIC, raw beef was stored on a wire shelf above cooked chicken.
- KDA_4_20211A2 - --A food contact stainless steel bowl had cracks around the rim. COS Bowl was discarded.
- KDA_6_30112 - --There was no paper towels at the handwashing sink near the back exit door. COS Paper towels were provided during inspection.
- KDA_6_501111A - --3 live roaches on a glue trap under the handwashing sink in the kitchen. Pest control last visit was 8/29.
- KDA_6_501112 - --Dead roaches were on a glue trap under the handwashing sink in the kitchen. Dead roaches were under the warewashing area, under the bar RIC, and in a empty aluminum can on the floor next to the wait saff soda machine.
- KDA_6_50116 - --A wet mop was stored in a mop bucket.
- KDA_7_10211 - --In the wait staff area, there was a bottle with clear liquid and a bottle of blue liquid inside. PIC said it was bleach water and glass cleaner. There was a white bucket of clear liquid and bubbles stored near the wait staff area. PIC said it was sanitizer (detergent and bleach).
- KDA_7_20111A - --In the wait staff area, 9 can of soda and a bottle of vinegar was stored next to chemicals. COS Soda and vinegar was removed.
- KDA_Defacto - --Fried rice and crab rangoons that was stored in the WIC, did not have a prepared date. PIC said it was cooked yesterday around 4:00pm, less than 24 hours. Egg rolls, and crab rangoons were stored in the deep fryer RIC, did not have a prepared date. PIC said it was cooked yesterday around 4:00pm, less than 24 hours. --Observed employee touch cooked rice with bare hand. However rice will have further cooking.
