KDA_4_60111A - --REPEAT: The ice bucket stored on top of the ice machine has sticky residue on the outside of the bucket. This residue was identified as remaining from when the bucket was originally purchased and was not cleaned off. Corrected on Site (COS) - the bucket was moved to ware washing by the manager. The white cooler near the back door (Kenmore) has a large accumulation of food crumbs/spill buildup in the door seal. COS - let the manager know this needed to be cleaned out routinely. She is having it cleaned.
KDA_6_501111A - --REPEAT: Below the 2-door windowed cooler by the back door in the kitchen there was one living roach. Note: The kitchen manager provided pest control records. The pest control company has made two routine visits in November (11/5 and 11/19) and have listed on their receipt that they are doing a routine maintenance for roaches.
KDA_6_501112 - --There are 50+ dead roaches below the 2-door windowed cooler by the back door in the kitchen.
KDA_Defacto - --The hot water at the hand sink in the kitchen is off. They are turning it on at the shut off valve below the sink when they need to wash their hands. This is being done because there is a leak from the faucet when the water is turned off at the handles.
