  • KDA_3_50116A2 - --There was a open bag of cooked hash browns being stored on a counter next to a electric skillet with a internal temperature of 58.1F. Employee said they were placed on the counter about twenty five minutes ago. COS Hash browns were placed back in the cooler.
  • KDA_4_20211A2 - --There was a plastic measuring cup stored as clean on a rack in the kitchen area that was melted on the outside of the measuring cup. COS Item was discarded.
  • KDA_4_50111A - --There are no handles on the steam table covers located in the kitchen area. The plastic strap on the door of the Inglis cooler is held together with duck tape.
  • KDA_4_50111B - --The door seal is broken on the GE cooler.
  • KDA_4_50112 - --There are two cutting boards stored as clean under the counter that have deep cuts and are pitted located in the kitchen area.
  • KDA_4_60111A - --There was a potato peeler, three measuring cups, and one metal strainer stored as clean on a rack had dried food debris on them from a previous located in the kitchen area. COS items were placed in the dish room for cleaning.

  • 4-302.14 - --There were no ph test strips for chlorine. COS-Person in charge brought some chlorine test strips.
  • 3-501.17(B) - --In the make table reach in, there was an open package of deli turkey with a date 8/13 and opened package of ham with a date of 7/31. Person in charge said these items were placed in the freezer on those dates and taken out on 10/9/15. COS-Person in charge dated these with dates of 10/14.
  • 4-102.11(B)(1) - --In the make table, there were two thank you bags storing tomatoes. In the inglish reach in lemons were being stored in a thank you bag. COS-items were moved to a food safe container.
  • KDA_2_30114G - --The cooked cracked raw shell eggs and then touched ready to eat shredded cheese with out washing hands and changing gloves. Educated on proper hand washing. COS-eggs and cheese were discarded.
  • KDA_2_30114I - --The cook touched his eye glasses and then touched lids to put on top of the cooked hash browns without washing hands.
  • KDA_2_40111 - --On a shelf in the customer area, there was a Styrofoam cup with a lid sitting over single serve napkins and wrapped silverware. The cup had coffee in it. The drinking part of the lid was open. COS-cup was moved to a table in the back.
  • KDA_3_10111 - --In the make table, there were two tomatoes stored in a thank you bag with mold and black spots on them with other tomatoes. In a container in the make table there were three tomatoes with black spots and mold stored with other tomatoes. In the Inglis reach in there were two lemon with black spots stored in a thank you bag with other. In the Inglis reach in there was a head of lettuce with green and yellow slime build up. COS-these items were voluntarily discarded. In the kitchen reach in there goulash was with out a date. Employee said it was made a week or a week and half ago. Also in the same cooler made on site chili was with out a date mark. Employee said he did not know when it was made. COS-items were voluntarily discarded.
  • KDA_3_30111B - --The cook touched ready to eat bacon with his bare hands. COS-bacon was discarded.
  • KDA_3_30414B1 - --Located on a shelf under the coffee pots, sanitizing cloth bucket had 0PPM chlorine sanitizer. There was also a wet cloth sitting a shelf next to the sanitizing bucket. COS-cloth was stored in bucket and sanitizing was corrected to 200PPM chlorine.
  • KDA_3_50116A1 - --Hot holding on the stove, hash browns 86.7F. Employee said they had been hot holding for half hour. COS-hash browns were reheated to 165F.
  • KDA_3_50116B - --Raw shell eggs in a cooler were at 50.9F, cooked said these had been out about half hour. COS-eggs were in the reach in cooler
  • KDA_3_50118A1 - --In the Inglis reach in, there was one piece of chocolate cream pie dated 9/22/15. Person in charge didn't know if that was correct date or new pie was placed in an old box. COS-pie was discarded.
  • KDA_3_50118A3 - --Cooked on site sausage crumbles was dated 10/12-10/19. Dated for more than seven days. COS- items were dated for seven days, 10/12-10-18.
  • KDA_4_20211A2 - --There was a lid on the cooked on site sausage that was melted and cracked. On the steam table, there was a lid covering the green beans that was melted. There was one metal bowl that was stored on a shelf that was pitted and scarred. Employee said they cook the sausage in it. COS-these items were discarded.
  • KDA_4_204112B - --In the Kenmore and Inglis reach in there were not any thermometers.
  • KDA_4_50111A - --In the GE reach in the seal is loose.
  • KDA_4_50112 - --The cutting boards are black and scarred.
  • KDA_4_60111A - --Under the steam table, there were two metal pans stored as clean with dried on food debris. Under the prep table on the west wall, there were two metal pans stored as clean with dried on food debris. Located on a knife strip, there were two pizza cutters and two knives stored as clean with dried on food debris. The magnetic knife strip had dried on food debris. On a shelf above the steam table, there were three serving spoons stored as clean with dried on food debris. There was a container of serving utensils with an accumulation of grease and food debris on the handles and food contact surface of the spoons. Above the prepping table hanging on hooks from a shelf, there were the following items stored as clean with grease and food debris on them, measuring cups and vegetable peeler. The interior walls of the microwave has an accumulation of food debris. Employee said they clean it daily, just didn't get done yesterday. There is one handle on the pop machine with and accumulation of dried on food debris.
  • KDA_4_60111C - --The handles of all the reach in coolers have an accumulation of food debris. The fan covers in the true reach in have an accumulation of dust. There was a container above the steam table that had food debris in it that was storing serving utensils. The shelves storing clean pans have an accumulaiton of food.
  • KDA_4_60211E4 - --The interior back and side walls of the ice machine have an accumuulation of black mold.
  • KDA_4_60213 - --The Kenmore reach in freezer has an accumulation of grease and food debris.
  • KDA_4_70211 - --At the ware washing area, the dishwasher was washing dishes. The ware washer had 0PPM bleach. COS-they washed and rinsed in the ware washing machine and sanitized in a bus tub.
  • KDA_4_90311B - --Located on a shelf under the prepping table, there were three pitcher that were stacked wet. COS-pitchers were placed at ware washing machine.

  • KDA_5_20511B - --A waitress washed a knife in the hand washing sink. COS-educated on hand washing sink usage.

  • KDA_6_30112 - --The hand washing sink did not have hand drying towels. You had to walk to the make table to get paper towels. COS-hand drying towels were moved closer to the hand washing sink.
  • KDA_6_50111 - --There are water damaged ceiling tiles in the kitchen by the waitress station. There is paint peeling on the wall behind the commercial dishmachine.
  • KDA_6_50112A - --In the wait station area, there are 10-15 old rodent droppings between the two reach in coolers. There is dirt and black mold along the baseboards in the kitchen. There is dried food debris on the walls throughout the kitchen.
  • KDA_6_50118 - --The hand washing sink, had an accumulation of grease and food debris. There was also a soiled towel in the basin of the hand washing sink.
  • KDA_7_10211 - --Hanging on the mop sink was a spray bottle with a yellow liquid in it without a label. Person in charge said it was degreaser. COS-bottle was labeled.
