3-302.11(A)(2) - --There was a container of raw chicken stored directly over a container of raw beef in the walk in cooler. COS- PIC moved all the chicken to the bottom shelf.
KDA_3_50118A2 - --There was an opened gallon of milk in the walk in cooler without any date marking. PIC stated the milk was opened yesterday 11/17 over 24 hours ago. COS- PIC correctly date marked.
KDA_6_501111A - --There was a live mouse in the back room. There was also fresh rodent droppings throughout the back storage area.
KDA_6_501111B3 - --There were no rodent control measures placed throughout the facility.
KDA_7_10211 - --There was a working spray bottle of purple liquid in the warewashing area without a common name listed on the label. PIC identified the liquid as degreaser. COS- PIC correctly labeled.
KDA_Defacto - --chicken 39 (walk in cooler) milk 41 (reach in cooler bar service area) --Chlorine measured at 100ppm in dishmachine, Chlorine test strips were provided. --There is a hose attached to the mop sink faucet that is hanging below the flood rim level. The atmospheric vacuum breaker (AVB) is not leaking
