COS- items discarded KDA_4_204112B - --affixed thermometer on cooler drawers does not work. PIC stated thermometers were on order. KDA_4_501114A - --Stored in sanitizer bucket, chlorine sanitizer liquid with a ph of between 10/20ppm. COS- sanitizer liquid changed out now reading 50ppm. KDA_4_50111B - --Seals on cooler drawers beneath stoves and flat top contain broken seals. PIC stated that seals are on order. KDA_4_50213A - --Single use ramekin used as portioning tool in mussel mix and 1 stored in mashed potatoes. COS- discarded. KDA_4_60111C - --Back of fryer encrusted with grease and dried food buildup. KDA_4_90111A - --Stored on top shelf of clean dishes rack, 2 plastic containers nested while wet. COS- separated. KDA_6_501111A - --7 live roaches crawling in kitchen areas (behind and bewlow make tables, and RIC) 1 live baby roach on WIC wall. KDA_6_50112A - --Floors beneath stoves, MT and dish machine soiled with dried food debris and grease. Located in the back storage room, grease build up on floor and in drain. Walls behind dish machine, MT, stove contain build up of dried food debris and grease. Walls behind handsinks soiled with dirt and dried food debris. COS- meat voluntarily destroyed. KDA_2_40111 - --Located in the downstairs kitchen area, behind closed doors, a room containing a chair and cigarette butts littering the ground. KDA_3_10111 - --Located in a large bin of corn starch 2 live roaches and 1 dead. COS- corn starch voluntarily destroyed. KDA_3_50116A2 - --Located in MT#1,container of caramelized onions with an internal temperature of 47.7 with a preparation date of 10/11 (temperature taken at 2:35).1 container of caramelized leeks with an internal temperature of 49.9,employee stated item was prepared 10/09. Located in lower portion of MT#1, container of mussel mix with an internal temperature of 46.3F employee stated item was prepared 2 days prior. COS- all items voluntarily destroyed. Vent hoods and filters encrusted with dried on grease and detritus. KDA_6_50111 - --At end of separation wall behind make tables, wall is torn from frame and bug device placed inside hole. Wall/floor juncture is pulled off at bottom of wall and bug capturing device is inserted. KDA_6_501111A - --90-100 live roaches found in various kitchen areas: top of freezer, in ceiling, in crevices of walls and door frames, inside motor of make table next to oven. KDA_6_501111B4 - --Located in back storage room with washer and dryer: old computer towers, taxidermy animals, card game supplies, a Christmas tree, stacked cardboard boxes with paperwork. Located in liquor storage area, old boxes that once contained liquor stacked along wall. Harborage conditions can lead to pests. KDA_6_501112 - --250-300 dead roaches on glue traps, fly strips and in water of steam table. KDA_6_50112A - --Floor beneath fryers, grill and oven are covered in encrusted food debris and detritus. Behind maketable 1,2 and 3 as well as large RIF there are potato chips and old food debris on the ground. Located in back storage room are two large tanks containing oil. One clean and one dirty. There is oil on the ground surrounding the tanks.
