4-302.14 - --There were no chlorine test strips available at the time of the inspection. The mechanical dish machine utilizes chlorine.
KDA_3_30212 - --On a top shelf there was a plastic food grade container with a white powder substance and it was not labeled. The person in charge said it was the breading flour for chicken.
KDA_3_50116A2 - --In a red crock pot there was shredded beef at 46.5F. The person in charge said it was placed out around 8:00am and it was tempted around 10:15am (less than 4 hours). The crock pot was not turned on. COS-rapidly reheated the shredded beef to 165F on the stove.
KDA_3_50118A2 - --In the Saturn RIC there was a plastic food grade container with sliced ham and was not date marked. The person in charge said it was placed in the unit on 12/28/15. COS-date marked the item.
KDA_4_20211A2 - --On a lower shelf and in a plastic tub, there were 4 rubber spatulas with cracked/rough edges (not easy to clean). COS-set aside to trim the edges smooth.
KDA_4_30212A - --There was no food thermometer available at the time of the inspection.
KDA_4_60111A - --On a magnetic strip there were 2 metal spatulas with dried food debris and grease build up on them and stored clean. COS-moved to the ware wash area. On a lower shelf there were 5 plastic containers with stickers/residue left on them and stacked clean (can hold bacteria). COS-moved to the ware wash area.
KDA_5_20515B - --The handsink in the kitchen leaks below and there is a bucket catching the water.
KDA_6_20215A3 - --The front door has a 1/4" gap along the bottom and the right side of the door. This could allow the entrance for pests.
KDA_6_50111 - --In the dry food storage room the wall insulation is exposed. The kitchen floor is cracked/chipping. There is a crack in the wall by the ware wash area and there is also mold on the wall. In the small dry food storage room there is mold on the wall towards the bottom (left hand side when walking in).
KDA_6_50112A - --There is trash on the floor in both of the dry food storage areas.
KDA_7_20111A - --In the kitchen there was a red sanitizer bucket with Quaternary Ammonia being stored directly on a metal food prep table. There was a knife, Pam spray and a metal bowl on top of the table and next to the sanitizer bucket. COS-removed the bucket.
KDA_7_20111B - --There was a red sanitizer bucket with Quaternary Ammonia being stored directly on top of the Whirlpool reach in freezer. COS-removed the bucket. There was one working spray can of glass cleaner being stored directly on top of the White Westinghouse reach in freezer. COS-removed the spray can.
KDA_7_20212A2 - --On the front counter there was a working spray can of Raid Flying Insect with an EPA Reg. No. 4822-569. The person in charge said they don't use it in the facility. They do have a contract pest control provider. COS-removed the can from the facility.
KDA_7_20612 - --There were 2 open bait traps being used in the dry food storage room located in between the restrooms. (Note: need to be tamper resistant control devices). COS-discarded.
