  • KDA_6_30112 - --No hand drying provisions at there the produce hand sink was to be located. Hand towels provided at the actual working area where produce is handled. Having to use the employee toilet room to wash hands at this time.
  • KDA_Defacto - --In compliance and gap at bottom of loading dock door is corrected. --In Compliance and the installation of a new and self closing door for the employee toilet room is complete and in good working order. --In Compliance by not storing raw meats over ready to eat. --In compliance with and provided ambient working ambient air temperature devices in the produce walk in cooler and at the front of the store where the bagged lettuce is located in an open case cold hold unit. --In Compliance with children's medications not expired, having been corrected at prior inspection. --In Compliance with the installation of florescent bulb protection above the produce working table.

  • 4-203.12 - --Ambient air temperature device located in the west wall open case that holds bagged lettuce has interruption of the fluid line giving unknown actual temperature of the unit.
  • KDA_3_30211A1b - --Packs of raw beef are stored on shelf above labeled fully-cooked smoked pork chops, located in the east open case retail meat cooler. Corrected by rearranging the raw to lower shelf.
  • KDA_4_204112B - --Ambient air temperature device, hanging directly under the condenser fan within the produce walk in cooler is so high up and darken glass as not able to be readable.
  • KDA_5_20212A - --Combination faucet is not equipped on what is described as a hand wash sink at the north corner of the storeroom which is where the produce cleaning and washing area is located.
  • KDA_5_20313A - --No designated mop sink in use as written directions are posted, at what is suppose to be the produce hand sink, directing to dispose of mop water in toilet or outside.
  • KDA_5_20411A - --Employee working with produce is forced to use employee toilet room as other sink in the corner area is not equipped properly to enable proper hand washing. Hose for filling mop buckets is hooked to the hot water faucet and appears that this is more a mop sink than a hand wash sink.
  • KDA_6_20211A - --two florescent tube light bulbs above the produce working table are not shielded or provided with protective covers.
  • KDA_6_20215A3 - --While there is not evidence of live or dead rodents or droppings, the south two-door loading dock has a 6 inch by 1/2 inch gap at the bottom of where the two doors close together.
  • KDA_6_30111 - --No hand soap provided for at what is suppose to be a produce area hand sink.
  • KDA_6_30112 - --No hand drying provisions located at what is suppose to be a hand wash sink for produce area.
  • KDA_6_50119 - --single employee use toilet room door does not self-close when not in use and open into the main area of the stock room.
  • KDA_Defacto - --hot water at meat department sink at 139 --meat department 3 compartment sink not set up for test. Department person described how this is set up. --Meat Department Deli and cheese retail unit for customer direct service is holding at 40, SE corner in retail area are 4 chest freezers with turkey and pizza all solid. South wall 23-door RIFreezers with all foods solid. North side of Isle 7 with 19 RIFreezers all solid. East end of Isle 7 4 ft open case with Reser's items and made in-house deli sandwiches at 39.2. East wall,north half of store, open case ranges 33-36 (raw meats/chicken). North wall 19ft(ham,bacon,lunchmeats ranges 33-35. North wall center section 36ft(eggs, cheese, yogurt,dairy ranges 33-38. West wall open case 16ft carrots/bagged lettuce at 41. west end cap of isle 3&4 with pizza at 31-36 range. Stockroom Walk in freezer foods solid.
  • KDA_KSA65_657b - --Six 4-fluid ounce children's ibuprofen and Imodium with expired dates of 6/15 and 10/15. Removed from retail sale.
