(Note: no leakage detected). There was also 4 cartons of raw hard shell eggs being stored in between ready to drink 1/2 gallons of milk, one bottle of Vanilla creamer and a bottle of Miracle Whip and mustard. COS-rearranged items. (Note: no leakage detected). In the General Electric refrigerator there was a carton of raw hard shell eggs being stored directly in between a glass dish of ready to eat gravy and a box of ready to eat individual packets of Mayonnaise. The carton of raw eggs was also being stored directly above to drawers of ready to eat individual packets of ketchup and mustard and ready to drink bottles of milk. COS-rearranged items. (Note: no leakage detected).
  • KDA_4_60111A - --To the right of the mechanical dish machine, on the clean dish side there were 2 black plastic holders (for individual sugar packets) with stickers left on the bottom (can hold bacteria). COS-removed stickers and put them back in the ware wash area.
  • KDA_4_90311A - --In the back storage room there was one large box of 16oz foam coffee cups being stored directly on the floor.
  • KDA_6_50111 - --There are 2 broken floor tiles at the front door.
  • KDA_7_30111A - --On a retail shelf there was one bottle of Ajax liquid dish soap being stored in between 2 packages of coffee filters and 2 boxes of Kleenex's. COS-removed chemical. (Note: no leakage detected).
