  • 3-302.11(A)(2) - --In the open air meat case packages of raw chicken were stored over raw salmon. COS, moved to an approved location.
  • KDA_3_30211A1b - --In the open air bacon case packages of raw bacon were stored on a shelf over RTE smoked sausages. In the open air meat case rolls of raw chorizo was stored over fully cooked ham steaks. Also, raw pork jowl was stored on a shelf over RTE ham. In the meat processing WIC a tray of raw beef was stored over a tray of RTE macaroni salad. Also,i n the meat processing WIC boxes of raw chicken were stored on a shelf over fully cooked ham. COS, all items moved to an approved location.
  • KDA_3_30511A1 - --In the produce area a bag of onions are stored directly on the floor. In the WIF on the north side of the building a box of whipped topping was stored directly on the floor.
  • KDA_4_60111A - --In the meat processing room a meat tray stored as clean had food debris on it. COS, moved to be cleaned and sanitized.
  • KDA_5_20314 - or, Installing an APPROVED backflow prevention device as specified under § 5-202.14.
  • KDA_5_20511B - --In the meat processing room there was a large styrofoam container covering the sink basin. COS, moved.
  • KDA_5_50117 - --The only restroom does not have a covered waste receptacle.
  • KDA_6_20211A - --The lights in the meat WIC are not shielded. The lights in the produce area are not shielded.
  • KDA_6_20215A3 - --There is a 1" gap at the bottom of the double doors on the northwest end of the building. There is a 3/4" gap on the bottom edge of the garage door The gaps allow the entrance of pests.
  • KDA_6_30112 - --In the meat processing room there were no paper towels at the handwashing sink. COS, employee supplied.
  • KDA_6_30114 - --There is not a sign to notify employees to wash their hands in the employee restroom. Inspector provided stickers.
  • KDA_6_50111 - --The sweep on the door to the meat processing room is bent and peeling off. In the meat processing room a ceiling tile above the prep table and single service trays is falling down. In the mop sink room paint is peeling off the walls and there is a hole in the wall. At the receiving dock there is exposed insulation on the walls. There is also exposed insulation around the ducts next to the produce area. The ceiling is sagging and falling down with exposed insulation over the produce area. In the storage area on the north side of the building there is various areas with exposed insulation. There are tiles missing in various places throughout the store.
  • KDA_6_50112A - --There is black build-up on the floor/wall juncture throughout the meat processing room. There also mold and a foul odor in the mop sink room. The seal on the door to the meat processing room from the retail area has black residue on it. There is dust on some items for retail sale throughout the store.
  • KDA_7_20111A - --In the produce area bottles of cleaner were stored on the drainboard of the 2-vat sink next to cans of rte tea. COS, tea.
  • KDA_7_20111B - --In the produce area spray bottles of cleaner and lotion were stored on a shelf over potatoes and tomatoes. Also, in the storage area on the north end of the building a pallet of laundry detergent were stored over boxes of hot cocoa mix. COS, moved.
  • KDA_7_30111B - --In the baby supply aisle on the retail shelf bottles of lotion, oil, and diaper rash ointment and cream were stored on a shelf over baby formula and pedia-lytes. COS, rearranged with all chemicals on shelves below the formula.
  • KDA_Defacto - --Note: In the meat processing room a box of aprons worn during processing was stored on a shelf next to bottles of cleaning chemicals.
