- KDA_4_60111A - --Hanging above the hand washing sink, there was one ice scoop stored as clean with food debris on it. Under the make take on the left end, there were six bowls stored as clean with dried on food debris.
- KDA_4_90111B - --Above the ware washing machine there were four metal bowls stacked wet. On a shelf next to the hand washing sink, there were five metal bowls stacked wet. In the north server area, there were about 20 bowls stacked wet. COS-these items were placed at ware washing machine.
- KDA_Defacto - --In the walk in cooler, cooked on site rice, corn and green onions 39.3F. North server station, salsa 51.2F. South server station salsa was 39.2F. In the make table on the north diced tomatoes 36.6, reach in were chopped peppers, onion and tomatoes at 37.3 On the South make table, chopped steak 39.5F, Reach in stuffed jalapenos 39.2F. On the table top make table, shredded cabbage was 40.1F. --In the walk in cooler, there was a container of made on site salsa that was at 58.5F. Person in charge said it was made today at 10:35, had a prep sticker on it with that day and time of preparation. In the north server station made on site salsa was 51.2F. Employee said it was made at 10:35AM. COS-items were moved to walk in cooler. Salsa is made from can tomatoes and not cooked. --On the steam table, cheese dip 170.2F. --The restroom faucets have been replaced with faucets you manually turn on and off. Water temperature at the hand washing sink in the women's restroom was 105.4F, in the men's restroom water temperature was 105.6F.

- KDA_3_30212 - --Sitting on the make table where the servers pick up prepared food, A squeeze bottle with yellow liquid was with out a common name. Person in charge said it was canola oil. COS-bottle was labeled.
- KDA_3_30414B1 - --On the cook line, a cook was using a moist towel to wipe off knife in between use. Educated on towel usage. COS-moist towel was placed in the sanitizer bucket.
