  • KDA_Defacto - --There was no evidence of roaches in the establishment. An employee said that there had been a customer that had complained that there was a roach on the window sill on Tuesday. Employee said that there was one roach on the window but that it had been killed. The person in charge (PIC) has contacted the pest control company who is scheduled to come in soon. The pest control company was last out on 10-8 and cited no evidence of pest activity then.

  • KDA_3_30212 - --Located on a shelf above the front counter, there were two unlabeled container of white powder which the PIC identified as salt. COS- labeled.
  • KDA_3_50116A2 - --Located at the make table (MT) of the kitchen, there was a container of pork and beef crumbles at 44F, a container of beef at 47F, a container of cut ham at 45F, a container of diced tomatoes at 49F, a container of cut bacon at 45F, a container of shredded asiago cheese at 49F, and a container of shredded parmesan cheese at 49F. Person in charge (PIC) indicated that the make table was stocked at 10:00 this morning. Ambient air temperature of the MT was 40.8F. COS- moved these food items to different reach in cooler (RIC). Educated PIC on keeping the MT closed when not in use and checking temperatures frequently.
