  • KDA_3_50116A2 - --In the Crosley refrigerator there was a plastic container of raw beef patties at 47.3F and 47.0F. The beef patties were out on the counter during lunch rush (out for 30 minutes). There was a plastic container of potato salad (made on site) at 44.1F. The potato salad had been out for the lunch rush (less than an hour). COS-placed the beef patties into the refrigerator and kept the potato salad container in the refrigerator. The ambient air temperature of the unit is reading 40.5F. On the counter there was a plastic container of sliced cheese at 66.9F. There was also a metal pan of raw beef steak at 69.0F. Both items have been out for 30 minutes due to the lunch rush. COS-placed the cheese back into the refrigerator and cooked the steak immediately. (Note: KDA 61 talked to the cook about placing the cheese on an ice bath to help keep it cool during rush hours).
  • KDA_4_10111A - --In the Crosley freezer there were 3 non-food grade Sterilite plastic containers being used to store biscuits, chicken planks and raw beef steaks. The food items were in direct contact with the non-food grade containers. COS-placed food on cling wrap and parchment paper.

There was also 2 cartons of raw hard shell eggs being stored directly above one raw ready to eat head of lettuce. COS-rearranged items. (Note: no leakage detected).
  • KDA_3_30211A1b - --In the Crosley refrigerator there was a metal pan of raw ground ground pork and an open package of raw bacon being stored directly above ready to eat individual packets of butter. There was a plastic food grade container of raw ground beef being stored directly above ready to eat flour tortillas. COS-rearranged items. (Note: no leakage detected).
  • KDA_3_50116A1 - --On the stove there was a pot of gravy at 110.3F. The cooked turned off the burner less than 2 hours. COS-reheated the product to 165F.
  • KDA_3_50116A2 - --On a metal prep table there was a carton of raw hard shell eggs at 67.1F. They have been out for less than 4 hours. COS-used immediately.
  • KDA_3_50118A1 - --In the Crosley refrigerator there was a bag of cut ham without a date. The cook said it was opened on 10/13/15 (past 7 days). There was also an open half gallon of milk without a date. The cook said it was opened on 10/13/15 (past 7 days). COS-discarded.
  • KDA_4_60111C - --There is grease build up on the hood filters above the stove/fryer.
  • KDA_4_60212B - --The microwave had build up dried food debris on the ceiling. COS-the cook cleaned it.
  • KDA_5_20515B - --The pipe below the handsink leaks when water is supplied. There is a "Do Not Use" sign on the middle stall in the women's restroom.
  • KDA_6_50111 - --One ceiling tile above the oven hood is missing and the insulation is exposed.
  • KDA_7_20111A - --There was a working spray bottle of Zep Fast 505 Industrial Cleaner & Degreaser being stored directly next to a bag of flour. COS-removed chemical. (Note: no leakage detected).
