- KDA_3_30211A1b - --Located in the reach in cooler (RIC), there was a package of raw bacon stored in contact with a package of cooked ready to eat pulled chicken. Corrected on site (COS)- moved bacon.
- KDA_3_50118A2 - --Located in the WIC, there were two opened containers of half and half with no date marks on them. The person in charge (PIC) said that they had been opened on 1/23. COS- dated. Located in the RIC, there was an opened package of commercially prepared hot dogs with no date mark on it. The PIC said that the hot dog package had been opened on 1/24. COS- dated.
- KDA_4_50111B - --The seal of the middle Frigidaire reach in freezer is torn.
- KDA_4_60111A - --Located on a rack behind the make table (MT), there were two pairs of metal tongs stored as clean that were soiled with dried food debris. COS- moved to wash area.
- KDA_4_60111C - --The fan guards of the WIC are soiled with dust. The outside of the three RIFs in the kitchen were soiled with grease. There is an accumulation of grease inside the doors and on the sides of the fryers. There is debris in the bottom of the Cres Cor warmer.
- KDA_4_90311A - --Located at the back of the kitchen, there was one box of single use plastic cups stored on the floor.
- KDA_6_20214 - --The employee restroom does not have a self closing door.
- KDA_6_50111 - --There are several stained ceiling tiles in the kitchen. The wall of the kitchen has paint chipping in places. There are a couple of cracked ceiling tiles in the kitchen.
- KDA_6_50112A - --The wall of the employee restroom/mop sink room is soiled with debris. There is debris on the wall of the kitchen. There is an accumulation of debris on the wall next to the kitchen hand sink. The floor under the hand sink and the prep table is soiled with debris. The floor under and behind the fryers is soiled with an accumulation of grease and debris.
