The meat tenderizer had dried residual food debris accumulation. A meat cleaver stored in a knife rack had dried residual food debris. COS: Equipment was removed from service to be cleaned/sanitized.

- KDA_5_20515B - --The middle vat of the 3-vat sink has a leak in the drain line that causes accumulating drainage liquid on the floor below.

- KDA_6_30111 - --The hand sink in the meat counter area is not supplied with soap. The hand sink in the men's restroom is not supplied with soap. COS: Soap provided.
- KDA_7_20111B - --In the dish/storage room various chemical cleaners are stored directly above ketchup. COS: Shelves were rearranged for proper storage. In the retail area two spray bottles of chemical cleaner are stored on food preparation tables above food contact surfaces and single-use food service gloves.
- KDA_7_20212A2 - --A can of "Hot Shot" insect spray, EPA # 46515-48-8845 is used in various area inside the establishment per the PIC and the product is not labeled as safe for use in a food establishment. COS: PIC removed product from establishment.
