KDA_2_40111 - --There was an employee drink without a lid stored on the dry goods shelf in the back.
KDA_3_50113A - --There was a package of RTE chicken pieces and a package of RTE veggie patties sitting out at room temperature. PIC stated both items were thawing and had been out for approximately 20-30 minutes.
KDA_5_501113 - --The lids to the outdoor trash receptacle were left open during inspection. There were several waste bags on the inside of the container.
KDA_6_30211 - --There was no toilet tissue available in the women's public restroom. COS- an employee restocked.
KDA_6_501110B - --There was an employee jacket on the dry goods shelf in the back on top of syrup in the boxes for the soda unit.
KDA_6_50116 - --There was a mop with a handle stored in the mop bucket not allowed to properly air dry.
KDA_Defacto - --meatballs 148 (steam table) --sliced ham 37 (make table) steak 41 (reach in cooler 1 front) lettuce 42 (reach in cooler 2 front) meatballs 40 (walk in cooler) --There were 6 containers of out of date QT test strips by the 3 vat sink. There was one left dated for expiration 12-15-15. Educated staff on expiration dates.
