KDA_7_10211 - --A spray bottle with a clear liquid lacked the common name. The person in charge (PIC) stated it was sanitizer. Corrected in site (COS) labeled.
KDA_8_10312B - --The facility is not following their HACCP plan. The last time the pH of the acidified rice has been documented to have been taken was 10/3/15. Also, in the rice acidification log on 9/21/15 the pH of the rice tested at 4.2. There is no corrective action documented.
KDA_Defacto - --3 compartment sink-200ppm Quat --Cooked rice room temperature storage log, cooler temperature log, frozen receiving log, pH meter calibration log, and brown rice monitoring log has never been filled out since their opening. They have been open since 8/11/15. The facility could not produce a HACCP plan or SOPs.
