- KDA_5_20213 - --At the mop sink, a red hose is attached to the faucet, and is hanging down, coiled in the bottom of the mop sink, below the flood rim, with no backflow prevention device or air gap. COS - removed hose from faucet

- KDA_6_50112A - --There is a build up of dust and debris on the ceiling vent above the shelving unit holding single-service plates and bowls.
- KDA_7_20111B - --A spray bottle of Window Cleaner is on top of a pop-in-a-box, which is stacked on top of another of the same, with canned foods on a rack below. COS - moved window cleaner spray away from food/beverages.
- KDA_7_20212A2 - --In the kitchen/warewashing area, three spray cans of Hot Shot Flying Insect Killer are found. Employee states that these are used to spray flies around the floor drain under the 3-vat dish sink, inside the building. EPA REG #46515-48-8845
