KDA_2_30114F - --Cookline employee cracked a raw, shell unpasteurized egg with gloved hands. Employee removed the contaminated gloves and put on a new pair of gloves without a hand wash in between tasks. COS education
KDA_3_50116A1 - --Made on site white gravy in steam table top at 118F. PIC stated it was made maybe 2 hours prior. COS added water to steam table top.
KDA_4_60211E4 - --Interior of ice bin and divider place of ice machine soiled with mold and rust buildup. Rust appears to be in direct contact with the ice on the side of the interior of machine.

KDA_5_20515A - --Two leaks in establishment. One leak under soda fountain area in front of house with mold growing on tile under soda fountain cabinet. Another leak in back kitchen area by handsink and where towels are stored. PIC stated she has been trying to get corporate to fix the leaks. Both leaks are causing mold to grow in both locations in the restaurant. The cold water is turned off in the back kitchen at the prep sink, the 3 vat sink, and the handsink due to this leak. The faucet is leaking at the 3 vat sink in back kitchen as well.
