Management awareness; policy present The establishment does not have an employee health policy or can not provide proof of policy. No signed employee interview and reporting forms exist regarding the prevention of the transmission of diseases through food by infected food employees. Note: Employee health forms given to the manager on duty at visit.
Insects, rodents, & animals not present Rodent droppings observed on display shelves, floor areas in the back storage room. Rodent damaged food products (bags of chips) observed at visit.

CFPM: Rolfe Ewing Kennard, FSP certificate #20996348, issue date: 07/09/2014 Facility will remain open for business to sell non-food products such as furnitures, toys, kitchen wares, etc. With the closure notice, facility has been ordered not to sell food products prior to resolving the rodent infestation problem, pay the re-inspection fee and approved by DC DOH inspector. License restoration procedure paper has been given to the manager at visit. Corrective plan of action for pest infestation violation has been given to the manager on duty.
SUMMARY SUSPENSION (In order for license to be restored. a non-compliance re-inspection fee of $100 [during normal business hours] or $400 [for non-business hours] MUST be paid prior to request re-inspection and all violations MUST be abated and approved by DOH.
