Employee replaced the wet rag in the sanitizer solution, and proceded to handle a RTE flatbread for a fajita sandwich without a glove change or a handwashing in between tasks. COS food discarded, glove change, handwash.
- KDA_3_10111 - --Opened package of RTE commercially prepared whole turkey chub in walk in cooler without an open date on packaging. PIC was unsure of open date but thought it may have been 12/13. COS discarded.
- KDA_3_50116A2 - --Sliced ham in large make table top unit at 46F, sliced tomato at 45F. Sliced roast beef at 53F. In reach in cooler underneath, sliced roast beef at 50F. Ambient temp of unit at 49.1F per lollipop thermometer. PIC stated the food had been in the unit since opening. COS lid placed back on make table top to cool product, food in reach in cooler removed to another cooler. Maintenance called.
- KDA_4_30111 - --Large make table/reach in cooler unit ambient temp at 49.1F. Cooler not capable of holding PHF at 41F or below.
- KDA_4_60111A - --Stored as clean on shelving for clean dishes in back kitchen area across from 3 vat sink, 6 metal pans with old date marking sticker residue on surface and one plastic container with visible food debris on surface. COS rewashed.
- KDA_4_60213 - --Backside of ice chute on consumer ice machine soiled with pink mildew buildup on both ice chutes.
- KDA_5_20515B - --Handsink in kitchen (sole handsink) at 71F to 85F when sinks are being filled for dishwashing. Water left on at mop sink at all times, AVB not designed to be under constant pressure. Water leaking from pipe mounted onto wall by ice machine. Water leaks directly onto the floor.

- KDA_7_20111A - --Bottle of dish detergent stored on prep table next to and touching a meat slicer in back kitchen area. Spray bottle of cleaner with bleach stored on prep table top next to a scale to be used with food and a box containing plastic food wrap. COS moved.
- KDA_7_20111B - --Bottle of dish detergent stored on shelf in back area of kitchen over boxes of soda syrup. COS moved.
- KDA_Defacto - --200ppm quat in sanitizer buckets in kitchen area. --Frozen foods frozen solid sliced tomatoes in make table top (small) at 39F, sliced turkey at 38F. Sliced roast beef in reach in cooler underneath at 40F. in walk in cooler, sliced turkey at 36F --handsink in toilet room at 103F --Roast beef in hot hold box at 145F --Strips in place for quat and chlorine.
