KDA_6_20215A3 - --The back doors in the storage room had a visible gap in the middle to the outside that does not protect against the entry of pests and rodents. The two front doors had a visible quarter inch gap at the bottom to the outside that does not protect against entry of pests and rodents.
KDA_6_501111A - --Fresh rodent droppings in the south fixture room along the wall and floor juncture. A recently trapped rodent was also in a trap in the same area.
KDA_6_501112 - --One dead rodent observed near the northeast corner of the storage room. One dead rodent in trap located in the south fixture room.
KDA_6_50112A - --Dried rodent droppings in the old men's toilet room, under shelving in on the sales floor and throughout the facility in the floor and wall junctures.
KDA_Defacto - --Mop sink faucet does not have backflow prevention but the hose attached to the faucet was air gapped. Would recommend a floor drain for mop sink and a faucet that has a built in Atmospheric Vacuum Breaker (AVB) to protect against backflow of toxins or contaminates into water supply.

KDA_6_20215A1 - --The northeast corner furnace room wall has crack with visible gap to the outside. Construction crew will work to fill the gap.
KDA_6_501111A - --Fresh rodent droppings were observed on the floor in the old downstairs toilet room on the eastside of the building and on the floor in the old south toilet room. Both locations were adjacent to open holes in the walls.
KDA_6_501111B4 - --The south toilet room (that is no longer in use and has no toilet) has holes in the walls allowing for rodent harborage. Construction crew will work to fill holes. The old downstairs toilet room has holes in the walls under the old sink and behind the toilet. There is also an old toilet the is no longer in operation allowing for the harborage of rodents. Construction crew was going to remove toilet and cap plumbing and fill holes.
