KDA_3_50118A2 - --Container of establishment made salsa, and package of cooked onions were in the reach in cooler without a preparation date on the food items. PIC stated the items were made 10/26/15. COS, all food items dated.
KDA_4_70211 - --PIC was cleaning food contact cutting boards in the 3 compartment sink. PIC did not use any sanitizer after cleaning the cutting boards.
KDA_6_20215A3 - --The back door has 1/4 inch gap space under the door. The space is large enough for pest to enter the establishment.
KDA_6_501111A - --12 fresh mouse droppings were on the floor of the north wall next to the grill. 30 plus fresh mouse droppings were on the floor behind a bag rice and under a food storage rack. PIC stated he does not have pest control provider.
