- 5-202.14 - --AVB at mop sink actively leaking. Water on at time of inspection. PIC called maintenance.
- KDA_4_60111A - --Stored as clean on shelving for clean equipment/utensils by walk in cooler, 2 plastic container with visible food debris on surface. Tomato slicer stored as clean on hook on same shelving with visible food debris on blade. COS rewashed.
- KDA_4_60213 - --Grease buildup on upper back surface of fryer. PIC stated they are cleaned daily. Small amount of syrup buildup on soda nozzles in consumer area soda fountain machines. PIC stated they are cleaned nightly.
- KDA_4_90311B - --Plastic containers stored as clean on sanitized drain board of 3 vat sink were stacked wet and not allowed to air dry.
- KDA_6_20211A - --Energy efficient light bulbs over self serve peanuts in shell and over ketchup dispensers in consumer area do not appear to be plastic coated nor shatter resistant.
- KDA_6_50112A - --Small amount of litter and dirt under ice machine. Small amount of litter, dirt buildup in cabinet under ice tea dispenser in consumer area.

Contracted pest control in place, invoice produced for last visit 12/3/15. --Establishment has a container of sanitizer to clean the shake machine that sits on a shelf by packages of napkins. Napkins are not considered a food contact surface, so discussed with PIC that the napkins are ok, but it can't be by gloves or paper towels. --Hamburger cooked at 178F --Handsink in kitchen area at 100F, handsink in female toilet room at 100F, handsink in sandwich prep area at 107F carbonator ok --Sliced tomatoes in walk in cooler at 40F, raw hamburger at 43F in walk in cooler. in make table top, sliced tomato at 41F in reach in cooler under make table top, sliced tomato at 38F in cold drawer 1, hot dog at 41F, in cold drawer 2, raw hamburger at 42F shake mix at 41F in shake machine. --Thermometers in reach in coolers are hooked up to the tablet in kitchen area. Tablet is used to read temperatures. If the ambient goes above 42F, the establishment is called by the head office. Per inspector, this counts for their ambient thermometers in the units.
