KDA_3_10111 - --One opened package of "muddy buddies" chex mix for retail sale in shopping cart for items for reduced sale. COS removed from retail sale, held for credit. KDA_3_20215 - --In shopping cart for "quick reduced sale" on the sales floor, 2 cans of peas and one can of green beans with dent to can severe enough to compromise the integrity of the contents as the dents were deep and/or on the seam. COS removed and held for credit. KDA_3_30211A1b - --On retail sales floor, raw chorizo stored next to RTE cheese and RTE lunchable meals in display case. In meat bunker display, RTE pulled chicken meal and pulled pork meal touching raw pork sausage and raw pork. Raw pork bacon stored on retail meat wall display case over RTE hot dogs. In meat cooler, 2 boxes of raw pork bacon stored on shelving next to and over boxes of RTE hot dogs. COS moved all KDA_4_20211A3 - --In produce area, counter is used as a cutting board and is covered in deep scoring. Not easily cleanable due to deep scoring. Recommended a cutting board for facility. KDA_4_50111B - --Seal to walk in (meat) freezer door is not intact. KDA_4_60111A - --Meat saw stored as clean with dried food debris on inside surface of saw. COS employee cleaned. KDA_4_60111C - --In meat room, packager is soiled with food debris buildup. KDA_4_60213 - --Fan covers in walk in cooler in produce area and meat room is soiled with dirt buildup. KDA_4_70211 - --Produce employee explained that he takes his paring knife home to wash it every night so it is clean for work. Knife is not being washed, rinsed, or sanitized. Discussed with PIC the need for training staff to use the 3 vat sink properly in the produce area. KDA_6_30114 - --No handwashing signage at any handsink in produce, meat room or any of the 4 bathrooms in establishment. Corrected on-Site, COS supplied KDA_6_50111 - --Floor directly outside meat room, behind the bunker is in need of repair. Light switch to walk in meat freezer is broken. KDA_6_501114A - --Back storage room is filled with unused equipment, storage units, shelving, etc. KDA_7_20111A - --In employee toilet rooms, various spray bottles of chemicals for cleaning stored on shelf next to paper towels for use at handsink for employees to dry hands at handsink. KDA_7_20111B - --In produce area, bottle of insecticide and bottle of sanitizer wipes stored on shelving over prep area for cutting vegetables/fruits, and sealing them in plastic wrap. COS moved KDA_7_20612 - --In compressor room off produce area, uncovered rodent bait in far north corner. COS removed. KDA_7_30111A - --On retail sales floor, bottle of lighter fluid stored on retail shelf next to boxes of single use silverware. Clorox wipes stored on retail shelf next to packages of single use bowls and plastic wrap for use with food. COS moved. KDA_7_30111B - --In produce area, bottle of cleaner with bleach stored on shelving over box of potatoes to be sold on retail sales floor. On retail sales floor, bottles of Clorox wipes stored on shelving over single use plates. In back storage area, bottles of motor oil stored over a box containing margarita mix. COS moved.
