- KDA_4_60111A - --In the top drawer of the make table, one slotted spoon was stored as clean with dried on food debris. COS(corrected on site)-spoon was placed to be washed.
- KDA_7_10211 - --Sitting on a shelf below the three compartment sink, a spray bottle with a greenish liquid in was unlabeled. Person in charge said it was not hers. She did not know what was in. COS-liquid was emptied and bottle removed from that area.

- KDA_4_60111A - --In a storage cabinet by warewashing machine, there were two plastic containers with sticker residue on them. COS-placed at warewashing machine. Stacked on a shelf by warewashing machine, there were 6 lids, 17 containers that had cracks along the food contact surfaces or corners broke off. There was one that had a hole through the contact surface of it. COS-items were discarded. In the white reach in, there was a container of salsa with the correct date, but it also had a sticker labeled cucumbers 10-7-41.OZ. Person in charge said the cucumber sticker should have been removed. COS-lid was replaced.
- KDA_4_60211E4 - --There is red mold on the plastic deflector in the ice machine.
- KDA_6_50111 - --There are stained ceiling tile in the following locations: four by the McCall reach in, warewashing area, one is moved and exposing ventilation pipes and about ten stained, one in front of the unisex rest room and one in front of the Vulcan grill. The food holding table on the serving line, is cracked on the top piece and in the middle there is a crack, they have tape along the rough edges.
