KDA_4_20211A1 - --There was deep scoring and pieces of loose shredded plastic on the interior of a white bowl used to chop salads. The bowl was stored as clean under the oven. "Corrected on site" (COS-removed from service).
KDA_5_20212A - --There was not water available at the hand washing sink by the 3-vat sink in the prep area. The water was turned off to the sink. Employees wash their hands at the hand sink by the make table, as per owner. There is a set of double doors separating the kitchen prep area and the make tables.
