- KDA_3_50116A2 - --In the East side of the WIC, sliced tomatoes measured 49.0F, cheese sauce measured 47.1F, chopped lettuce measured 51.3F, shredded cheddar cheese measured 45.7F, blue cheese measured 45.5F, raw bacon measured 45.7F, and sliced cheddar cheese measured 45.5F. All above mentioned potentially hazardous foods (PHFs) were in the cooler more than 4 hours. COS-voluntary destruction (VD), moved other PHFs to the West side of the cooler (there is a half wall dividing the sides of the cooler) as it can keep the food 41F or below. There needs to be a permanent solution as it is a reoccurring issue. PIC placed work order.
- KDA_4_30111 - --The East side of the WIC is not holding PHFs at 41F or below. The ambient temperature measured 46.8F. PIC placed work order
- KDA_4_60211E4 - --The pop nozzles have a biofilm on them. Advised to clean.
- KDA_4_70211 - --The 3 compartment sanitizer is below 200ppm quaternary ammonium. Dishes were being washed during the inspection. COS-added more sanitizer to sink (200ppm quat)
- KDA_5_20515B - --At the mop sink, there is a grey foul smelling liquid in the basin of the sink. When inspector turned on the mop sink dispenser liquid, the liquid did not drain. The PIC stated the mop sink does not drain. The hot water knob of the mop sink is also missing. Advised to fix mop sink.
- KDA_6_50112A - --The floors in the kitchen (especially under equipment) have a lot of foul smelling old food,grease and dirt. Advised to clean out as it is a pest attractant.
- KDA_8_30411A - --The license is not posted for public viewing.
