- KDA_4_60111A - --Numerous stored as clean and in use single use plastic food containers have stickers on the lids. A single use plastic pickle bucket re-used for foods has a sticker on the outside of the bucket. Soil and grime on a cutting board and on the make table cutting board.

Produce should be washed in the sanitize vat. As per the KDA office, at least one vat must be indirectly plumbed. If only one vat is changed, change the sanitize vat.

- KDA_6_30112 - --At the start of the inspection, no paper towels at the kitchen handsink and no paper towels at the employee restroom handsink.
- KDA_6_501111A - --Fresh rodent droppings (number approximate): 30 on the floor in dry storage by the water heater tank and shelf next to the tank. As per the person in charge, they have a hired pest control service that sprays for insects, and sets out mouse traps. Pest control method observed was mouse traps.
